Abstract

Maternity intensive care nursing is the field
of nursing with a focus on the utmost care of the critically ill
or unstable patients. Critical care nurses can be found
working in a wide variety of environments and specialties,
such as emergency departments and the intensive units
(Johnson, 2013).
An important function of critical care nursing is to
provide continuous observation of critically ill patients.
Observation will reduce a patient‘s risk of precipitous
deterioration, monitor their total dependence on support
equipment and prevent their agitation or confusion leading to
harm (Ball, 2002).
The Aim of the study: To evaluate the effect of maternity
intensive care guideline on nurses‘ knowledge and practical
skills.
This aim was achieved through following:
1- Assessment of nurses‘ knowledge and practical skills at the
maternity intensive care unit
2- Design and implement guideline for nurses at maternity
intensive care to enhance their knowledge and practical skills.3- Evaluate the effect of maternity intensive care guideline on
nurses‘ knowledge and practical skills.
4- Dissemination the result of this study
Research hypothesis: An instruction guideline will enhance
nurses‘ knowledge and practical skills in maternity intensive
care unit.
A Quasi- experimental study was utilized, at intensive
care unit, in Ain Shams University Maternity Hospital for four
days per week (two days from 8 am to 2 pm and two days from
2 pm to 8 pm for seven months. A convenient sample was all
nurses (40 nurses) who were worked and accepted to participate
in study, at previously mentioned study setting.
Tool was used for data collection; 1) A Structured
interviewing questionnaire schedule which included two parts:
The first part: assessed nurse‘s general characteristics. The
second part: assessed nurses‘ knowledge regarding nursing care
integrated at maternity intensive care unit
2) Observational checklist: it consisted of all nursing
procedures which were done by nurses in maternity intensive
care unit A Supportive instruction guideline was designed by researcher
according knowledge and practical skills related to critical ill
mothers in maternity intensive care unit.
The pilot study was conducted for 10 %( 4 nurses) of total
sample at selective all nurses who were worked at previously
mentioned study setting
Main results of the present study were as follows:
 There was highly statistically significant between pre &post
immediately, and post three months of intervention related to
nurse‘s Knowledge about maternity intensive care.
 The main source of nurse‘s Knowledge about maternal
critical patient was work experience represent (72.5%),
followed by the study and Training course (65% & 22.5%)
respectively.
 There was highly statistically significant between pre &post
immediately, and post three months of intervention related to
total score of nurse‘s Knowledge about maternity intensive
care.
 There was highly statistically significant between pre
&post immediately, and post three months of
intervention related to general care of nurse‘s skills (p value ≤ 0.001) and statistically significant with
assessing patient condition(p< 0.05)
 There was highly statistically significant between pre &post
immediately, and post three months of intervention related to
Nursing Care of Patient on ventilator, and Care of toxemia
of nurse‘s skills, (P value ≤ 0.001).and statistically
significant with Care of patient Hemorrhage and Care of
diabetic patient (P < 0.05).
 There was statistically high significant relation between pre
&post immediately, and post three months of intervention
related to total score of nurse‘s skills(P value <0.001).
 There were statistically no significant relations between
Nurses‘ Knowledge and general characteristics (age,
Qualification, Years of Experience and Training course) preintervention
(P value >0.05).
 There were statistically high significant relations between
Nurses‘ Knowledge and general characteristics (age,
Qualification, Years of Experience and Training course)
post intervention (p-value ≤ 0.001).
 There was statistically significant relation between Nurse‘s
skills and general characteristics (age, Qualification and
Years of Experience) pre-intervention (P value < 0.05).There were statistically high significant relations
between Nurse‘s skills and general characteristics (age,
Qualification and Years of Experience) post intervention
(p-value ≤ 0.001).
 There was a highly significant positive correlation
between nurses‘ knowledge score and nurses‘ skills score
about maternity intensive care regarding their post three
months of intervention) (P value ≤ 0.001).
 There was statistically significant relation between
general characteristics (pre-& post immediately, and
 Pre-and post-three months of intervention related to
Nurse‘s Knowledge and Nurse‘s skills about maternity
intensive care (P < 0.05).
In Conclusion &Recommendations
In conclusion, the present study and an instruction supportive
guideline about maternal intensive care unit had significant
improved post intervention compared to pre-intervention.
Based on the results of the present study the following
recommendations are suggested:1. Refreshing courses pre-service and in-services training
programs to enhance nurse‘s practical skills at maternity
intensive care units.
2. Specialize intensive care unit nurses must be available at
maternal intensive care unit.
3. Established Standards and protocol of care for maternal
intensive care.
