Abstract

Sacrococcygeal pilonidal disease is a common and well recognized entity.
It mainly affects young aged males after puberty especially patients with deep natal cleft.
The etiology of Pilonidal sinus disease is not fully understood, however, the most important cause is believed to be fallen hairs accumulating within skin folds and implanting within the skin causing infection.
Although it is clinically asymptomatic in some cases, PSD may also present as a chronic, complicated disease, characterized by multiple sinus tracts, leading to severe impairment of patient quality of life. Various approaches have been proposed to decrease the risk of development of chronic pilonidal sinus following the treatment of pilonidal abscess.
The ideal surgical technique is controversial, many surgical procedures have been used to treat this condition ranging from open drainage or the treatment using a flap and the use of offmidline flap is better than midline ones.
The Karydakis Procedure is used for the cure of pilonidal sinus when it is appropriate. A drain is commonly used under that flap also. The procedure results in a wound that is offmidline. Also sometimes tie over compressing sutures could be used instead of the drain as the drain may cause the introduction of organism to the wound cavity.
The aim of this study is to compare the clinical outcome of the standard Karydakis procedure (using a drain) vs Karydakis technique using Tie-over compressing sutures.
In this study, 40 patients were randomly allocated into 2 groups by closed envelope randomization technique. 20 patients were treated using standard Karydakis procedure (using a drain) and the other 20 patients were treated using Karydakis technique using Tie-over compressing sutures.
This study showed that the treatment with the karydais procedure using the drain led to a hospital stay where 16 patients spent 1 day and 4 patients were hospitalized for more than one day and the seroma was found in 1 case.
