Abstract

VII. Summary & Conclusion Glaucoma is a major public health problem emerging as the second most common cause of blindness in the world and the major cause of irreversible blindness worldwide. According to the study by Traynis et al, 53% of patients with glaucoma with 24-2 Humphrey VF had VF defects in the central 10 degrees. These results emphasize the importance of a self-assessment test for detecting glaucomatous VF defects in central area near fixation [6]. VF testing has been utilized for a long time to detect defects, determine the specific pattern of visual field loss for diagnostic purposes, and monitor patients for evidence of visual field progression. The detection of visual field abnormalities in glaucoma is an indicator of optic nerve damage [7]. However, it has been recognized that perimetry is a relatively insensitive test, in that it detects glaucomatous damage only after a significant proportion of nerve fiber bundles already have been damaged. So, many patients with glaucoma frequently go undiagnosed until severe bilateral vision loss has occurred [9].Amsler grid has an important role in evaluation of the central 10° surrounding the fixation point. The test consists of 7 charts that are essential in the analysis of the visual function. It helps in early evaluation of functional maculopathies, together with glaucomatous visual field defects and optic nerve diseases. Amsler grid test may be useful in detecting undiagnosed glaucoma patients, especially in the moderate to severe stage of the disease. It is simple test (test time <1 minute per eye in the present study), inexpensive and it is the only portable visual field testing suitable for mass screening
The size of the Amsler grid scotoma had a strong correlation with 10-2 VF parameters (MD, Scotoma extent). This means that Amsler grid testing may have a role in monitoring glaucoma progression within the central 10 degrees VF. Furthermore, it should be noted that because of its high specificity and
positive predictive value (100%) for both, it produces a very low number of false-positives. This prevents patients from becoming falsely alarmed or seeking unnecessary medical care.
