Abstract

The present study was carried out at the Family Planning Clinic of the Department of Obstetrics and Gynecology of Berket El Sabea Hospital from November 2017 to June 2018 on 147 participants who are coming for intrauterine device (IUD) insertion.
Three vaginal swabs were taken from posterior vaginal fornix before insertion of IUD, one month and two months after its insertion for detection of bacterial vaginosis.
Detailing questionnaire include :demographics, symptoms & diagnosis of BV, first episode of BV, recurrent episode of BV, possible prevention & treatment options was filled out by each participant during the first visit.
Proper history of complication of IUD include the degree of pelvic pain, presence of fever more than 38°c, heavy vaginal bleeding, expulsion of IUD and abnormal vaginal discharge was taken during the second and third visit one and two months after insertion of IUD.
Participants with positive first vaginal swab were treated and followed up for detection resistant cases for BV.
The present study revealed that the incidence of bacterial vaginosis was 27.2 % before insertion of intrauterine device and 9.5 % new cases after insertion of intrauterine device.
The incidence for response to Metronidazole was 100% and four cases were relieved spontaneously without treatment.
The commonest complications in the second visit were pelvic pain (23.2%) then abnormal vaginal discharge (19%), followed by vaginal bleeding (18.4%) and dyspareunia (15.6%) while the least complications were itching (6.1%) . Fever didn’t occur in any patient.
Pelvic pain, abnormal vaginal discharge and dyspareunia were significantly higher in the first swab positive gram stain group than the negative gram stain group.
