Abstract The cornea is the main refractive surface of the eye and also represents a barrier against environmental insults. Because of the risk that it may permanently impair vision or perforate the eye, a corneal infection is considered an ophthalmologic emergency. Plant ocular trauma and diabetes are considered main risk factor for fungal keratitis. This study was carried out on 30 patients with resistant corneal ulcers attending to Mansoura ophthalmology center outpatient clinic: during period July 2017 till June 2018.The present study depended on both laboratory and clinical diagnosis. The study was performed to compare topical amphotericin B (AMB) eye drops in group (A) and combination of topical AMB eye drops with subconjunctival fluconazole injection in group (B) of patients. Regarding healing of corneal ulcers; the percentage was higher in group (B) treated by AMB eye drops with subconjunctival fluconazole injection. Fungal Keratitis has been proven to be a continually challenging ocular disease for patients, healthcare providers, and society. The most common risk factora have been identified, diabetes and plant ocular trauma. Laboratory diagnosis should be started before treatment which is important for early detection , management and less complication of fungal keratitis. The study stated that efficacy of dual therapy of topical amphotericin B (AMB) eye drops with subconjunctival injection of fluconazole was higher than efficacy of topical AB eye drops alone. Larger nationwide study over along period of time is recommended for more confirmation of privilage of subconjunctival injection of antifungal over topical antifungal in management of fungal keratitis. |
