Abstract

Kidney transplantation may be followed by a number of complications of parenchymal, urological and vascular nature. Early diagnosis of these complications may determine the survival of the graft. As graft recipients are fragile patients and may need decisive studies for fear of allograft failure, therefore the diagnostic tool used to assess allograft dysfunction should be noninvasive as possible. This study was aimed to evaluate the role of ultrasonography, color and power Doppler and sonoelastography in early diagnosis and follow up of post renal transplant complications. This study performed in Radiology department, Urology and nephrology center Mansoura University in the period from 10/2017 to 10/2018, all transplanted patients underwent complete history taking, full abdominal and pelvic examination then examined by routine ultrasound, Color Duplex Doppler and Sonoelastography by using multifrequency ultrasound system (IU22, Philips medical system, Hollande, the Netherland) with 1-5 MHz transducer. Our 40 patients included in this study were biopsied after doing Doppler US and elastography, there were 13 normal patients (32.5%), 13 ATI patients (32.5%) and 14 CTI patients (35%) All 40 patients 26 males (65%) and 14 females (35%) with age ranged from 10 – 52 years old with mean age of 32.03±9.9 years’ old Ultrasound done for all 40 patients, 3 of them showed minimal perigraft free fluid (7.5%), 2 had simple parenchymal cyst (5%),3 was GII echogenicity (7.5%),4 showed small stones (10%),2 show perigraft collection (5%).,1 had heterogeneous collections at pelvis (1%) and the others were normal (62.5%). By Color duplex Doppler we found 26 patients with good cortical perfusion (Grade 0) ,13 of them normal and 13 were acute, 12 patients’ reasonable cortical perfusion (Grade 1) were chronic and 2 of them had patchy perfusion (Grade 2) were chronic. By sonoelastography the mean elasticity values of normal patients (13 patients) were 5.7 with range (3.99-9.91) while the mean elasticity values of the patients had ATI (13 patients) was 18.91 with range (18.24-24.52) and in patients had CRI was 30.62 with range (30.32-36.90). The statistical analysis as regard the difference between the means elasticity values in differentiation between the three studied groups in our thesis, it was highly significant (p value <0.001). So it will increase the diagnostic performance of renal transplant us. The combined use of B-mode US, color duplex Doppler and sonoelastography improved early evaluation post kidney transplant complications
