Abstract Summary from July 2017 to January 2018 a comparative, prospective Case- Control study is carried on 20 patients with severe osteoarthritis, who underwent primary TKR in order to evaluate the effect of IA-TXA injection on decreasing the amount of blood loss after the operation. The patients were divided into two groups, the 1st group (10 patients) injected with TXA and the 2nd group (10 patients) injected with normal saline. Inclusion criteria: Adult patients underwent an elective primary unilateral TKA for advanced osteoarthritis of the knee joint. Exclusion criteria: 1. Post traumatic TKA, revision TKA. 2. Preoperative use of anticoagulant therapy within five days before surgery. 3. History of arterial or venous thromboembolic disease. 4. Abnormal bleeding tendency or bleeding disorder. 5. Pregnancy, breastfeeding, anemia and major comorbidities. 6. Contra-indication for TXA use ( active intravascular clotting process, acquired defective color vision, subarachnoid hemorrhage, hypersensitivity to TXA, and no any of history of serious adverse effects, and hematuria). Method: A similar protocol of surgical management was used for both groups. All patients were operated under epidural anesthesia, with tourniquet control of 350 Hg pressure.
