Abstract
Summary and Conclusion
We studied effect of iodinated contrast media administrated in 85 patients during elective coronary angiography on thyroid functions in the cardiology department of Beni-Suef University Hospital in the period from December 2017 to September 2018.
All patients under the study were subjected to full history taking, clinical examination (Thyroid Examination) and laboratory investigation (Baseline TSH& FT4 and at 3& 6 months after coronary angiography).
Our study showed that there was a statistically significant increase of TSH level from baseline till 3 &6 months (P-value=0.007 & 0.002), and the incidence of increase is 2.4%&10% at 3&6 months.
There was a significant association of ionic contrast with the higher level of TSH after 3 months (P-Value=0.013),also increase the level of TSH after 3 and 6 months occurred more with patients of GFR less than 60 (P-value=0.049 and 0.45) respectively.
Our study limitations were, 1) Measurement Free T3 &TPO weren’t done due to financial issues, 2)Loss of follow up of some patients after 6 months. So, we recommend 1)Introduction of larger number of patients for more longer periods with different age categories and different stages of renal dysfunction taking into consideration our study limitations,2)Genetic studies to demonstrate individual sustainability.
Conclusion:
Administration of Iodinated Contrast Media (ICM) associated with thyroid dysfunction mainly hypothyroidism (Subclinical or Overt ) so we should closely monitor patients after receiving ICM especially who have thyroid dysfunction and/or renal impairment.
