Abstract

This study was performed to determine the effect of local injection of Platelet-rich plasma (PRP) in patient’s mild and moderate knee osteoarthritis (OA) and its effect on patient’s quality of life (QOL) and pain in comparison to corticosteroid injection
This study involved 50 patients were suffering from mild to moderate primary osteoarthritis of knee joint, diagnosed clinically, and by plain x- ray. They were 42 (84%) females and 8 (16%) males. Their aged ranged from 40-70 years with a mean ± SD of 50.4±8.7 years.
Patients are diagnosed OA knee diagnosed according to the American College of Rheumatology (ACR) criteria for the classification of osteoarthritis of the knee.
All patients were subjected to the following:
*Full history taking,
*Full detailed clinical examination
* Laboratory assessment
* plain x ray for knee
*Assessment Questionnaires: Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC)
* Assessment Questionnaires: Visual analogue scale (VAS) Randomization in a double blind way, patients were divided into: 1) 25 patients were injected intra-articularly with about 3-4ml of PRP for one affected knee. 2) 25 patients were injected intra-articularly with 2ml triamcinolone acetonide for one affected kne Then, they will be re-evaluated again after 1 month of injection and after 3 months
The results showed the following:
 There was no significant difference in patients age groups, BMI and sex.
 No significant difference in radiological grading of both studied groups
 shows no statistical significant difference in pre injection between the two groups in total WOMAC score
 Post injection there was statistically highly significant difference between two groups in total WOMAC score with more decrease in PRP group
 In follow up after 3 months there was high significant difference between two group in total WOMAC score with more decrease in PRP group
 In PRP group there was significant difference from pre injection to post injection in pain score also there was significant difference from pre injection to follow up after 3 months
 In post injection and follow up there was significant difference between both groups, with decrease pain in PRP more than triamcinolone group.
 In post injection both groups show significant difference from pre to post injection & also there was significant difference between pre to follow up after 3 months regarding stiffness but by comparison of both groups there was significant difference in post injection between PRP & triamcinolone and there was high significant difference in stiffness in follow up between PRP group & triamcinolone group  In PRP group there was significant difference between pre- to post injection also there was significant difference between pre- to follow up after 3 months of injection
 In other group of Triamcinolone there was significant difference between pre- to post injection also there was significant difference between pre- to follow up after 3 months of injection
By comparison of both groups:
 No significant difference between both groups in pre injection in degree of difficulty.
 There was significant difference between both groups in post injection and significant difference in follow up after 3 months
 There was no significant difference between both groups pre injection. Both groups recorded high significant decrease in VAS scale readings after injection.
 In PRP group there was highly significant decrease in VAS scale readings in follow up after 3 months of injection.
 Also in triamcinolone group there was significant decrease in VAS scale readings in follow up after 3 months of injection.
By comparison both groups recorded highly significant decrease in VAS scale readings post injection & in follow up; more decrease in in PRP group.
