Abstract

This study was a clinical prospective comparative study that was designed to evaluate and compare the efficacy of intrathecal hypobaric Lidocaine 0.5% versus Isobaric Lidocaine 2% in Perianal fistula surgeries requiring spinal anesthesia and the patients were randomly divided to two groups 20 patients in each group. Patients received 8 ml (0.5%) Lidocaine and another group received 2ml Lidocaine 2%.
We observed that:
• As regard Onset of sensory block. Intrathecal hypobaric Lidocaine had a faster onset than Intrathecal Isobaric Lidocaine. However ; Duration of Sensory block and Time needed to reach Highest level of Sensory block were longer in Isobaric lidocaine group,
• As regard Onset of motor block. Intrathecal hypobaric Lidocaine had a faster onset than Intrathecal Isobaric Lidocaine. However; Duration of Motor block was longer in Isobaric lidocaine group.
• As regard Systolic Blood pressure; there was significant decrease of systolic blood pressure immediately after injection in both groups with peak decrease at 5min recordings group and with peak decrease immediately after injection on second group.
• As regard Disatolic Blood pressure; there was no significant decrease of diatolic blood pressure after 5 minutes in both groups but there was decrease after 10minutes in group 1 more than group 2
• As regard Heart Rate there was significant decrease in hypobaric Lidocaine than Isobaric Lidocaine group immediately after injection; Peak DROP was after 10 min in first group while it was after 5 min in second group.
• As regard Oxygen Saturation there was no Statistically Significant difference between the two groups.
• As regard Complication that No Significant difference Between Two groups in rate of Complications; Hypotension was most frequent complications in the two groups.
In conclusion:
The use of hybo baric lidocain reveal early onset for sensory and motor block with early recovery and hemo dynamic stability than iso baric lidocain in short stay surgeries as perianal surgeries .
Recommendation:
Compare between hypo and iso baric lidocain with the same volume and concentration.
