Abstract

Sexual dysfunction is a chronic complication of diabetes mellitus. It is reported that especially the duration of diabetes, glycemic control, age, and the pharmacological treatment used are associated with the sexual dysfunction. Diabetic patients are under the threat of vascular and neurologic complications and psychological problems. This situation exposes them to the risk of organic and psychological sexual dysfunction. However, the sexual dysfunction among women has been ignored for many years (Mutlu Şahin & Zümrüt Akgün Şahin, 2015).
The popular media and society are often sources of misinformation and distorted ideas about sex. Health providers must be ready to identify and dispel myths about sex that can negatively influence sexual behavior. Having anatomic diagrams and models can help patients gain a better understanding of their bodies and provide them with the vocabulary and knowledge to have effective and meaningful discussions about sex and sexual health (Sheryl A. Kingsberg, and Terri Woodard, 2015).
A sexual counseling model for care providers, such as the PLISSIT model, might be a useful tool to improve the discussion of sexual issues in patients with diabetes mellitus (Ayaz S, 2013). This model allows primary care providers to start a discussion regarding sexual issues with individuals and during the next steps of model, useful information and suggestions to be incorporated in the care plan (Nabila El- Sayed Saboula, Marwa Ahmed Shahin, 2015).
The present study aimed to evaluate the effectiveness of the adaptation program for diabetic women about sexual dysfunction through assess sexual problem among diabetic women by using Fe¬male Sexual Function Index (FSFI), assess knowledge, attitude and practices regarding sexual problems among diabetic women, and apply (PLISSIT) model on diabetic women with sexual problems.
The study was conducted at diabetes outpatient clinic at Ain Shams University Hospitals, Cairo, Egypt. The sample size was 105 women attending for diabetic outpatient clinic at Ain Shams University Hospitals. Women selected according to inclusion criteria, 5 women DROP of the study. The sample criteria were (married, educated women, with reproductive ages group (18-48 years) diagnosed as diabetic patient, sexually active for at least four weeks ago. In addition, sample women reported to have sexual problems.

The data were collected through:
Three types of tools were used for data collection Structured interviewing questionnaire was designed by the researcher after reviewing the relative literature, Pre - post assessment sheet for knowledge, attitude, and practices of diabetic women, and Female Sexual Function Index.
Results of the present study revealed that:
• Sexual dysfunction in diabetic women were statistically significant with age, education, duration of marriage, parity, history of medication uses, glucose control, and diabetic end-organ complication.
• Most of studied women were have unsatisfactory knowledge, attitude, and practices about sexual dysfunction but there is highly statistically significant regarding change of knowledge, attitude, and practices of studied women through program utilization.
• The adaptation program has positive effect on women’ knowledge, attitude and practices towards different aspects of sexual issues among diabetic women.
The present study recommended to:
• Increase awareness of diabetic women through classes, sessions, and educational programs in outpatient clinics to discuss sexual dysfunction related to diabetes mellitus.
• Integrate and enhance role of nurses in outpatient clinics as a health educator through periodic training program to help in early detection of sexual dysfunction and prevention through health teaching.
• In addition, prepare a secure environment in the hospital to discuss sexual problems with women freely.
• Further studies must be done in large sample in different hospitals to better understand the relationship between diabetes and sexual dysfunction.
