Abstract

Low back pain (LBP) is a common problem among pregnant women, the prevalence of LBP during pregnancy about (70-85%) of women who experience LBP, about(80%) of pregnant women with LBP experienced a serious problem in everyday activities, housework, and their job which lead to decrease quality of pregnant women. LBP may be due to combination of mechanical, hormonal, circulatory psychosocial factors (Joanne et al., 2015).
Strategies to manage pregnant women‘s low back pain and disability are varied and include different kinds of treatment modalities such as physical exercise, transcutaneous nerve stimulation and acupressure. Physical exercise during pregnancy can restore optimal biochanics, that will strength back muscles, prevent gestational diabetes and excessive gestational weight gain. Acupressure is one of the treatment modalities that can be effective in relieving low backaches, muscle aches, and tension due to stress. There are great advantages to use acupressure as a way to balance the body and maintain good health, increase circulation and enable body to relax deeply, acupressure resistance to disease and promote wellness (Hayden, 2015).The present study aimed to evaluate the effect of acupressure therapy versus exercise on low back pain among pregnant women through the following;
 Evaluate the effect of acupressure on reliving functional disabilities and low back pain for the pregnant women.
 Evaluate the effect of exercise on reliving functional disabilities and low back pain for the pregnant women.
 Compare the effect acupressure versus exercise on reliving functional disabilities and low back pain for the pregnant women.
The study was conducted at the antenatal clinic of Ain Shams Maternity University Hospital. Purposive simple randomly sample technique was used to recruit two hundred thirty two women in the study those randomly assigned into two groups. group (I) consist of (116) women with functional disabilities and low back pain during pregnancy who received exercise. group (II) consists of (116) women with functional disabilities and low back pain during pregnancy who received acupressure. The period of implementation was 7months started from the be gaining of April 2015 till the end of October 2015.
Three tools of data collection were used.
Questionnaire sheet: was used to obtain the socio demographic data, and past experiences with low back pain during pregnancy. Arabic version of Quebec back pain disability scale (QDS) was used to measure the level of functional disability. Arabic version of RMDQ was used to assess limitations in different daily life activities, pain sleep, appetite and emotional function due to back pain.
The study revealed that:
 Nearly two thirds of pregnant women in exercise group their age among 20 <30 years. While, half of pregnant women in acupressure group their age among 30- <40 years.
 As regards education more than one third of women in exercise group and one fifth of women in acupressure group had university education.
 Majority of women in exercise group had back pain in previous pregnancy while all of women in the study group had back pain in previous pregnancy.
 More than one third (38.7%) of acupressure group had knowledge about way to relive back pain than exercise group with highly statistically relation.
 The mean of pre Roland disability 59.99±21.8 in exercise group mean while in acupressure group 70.72± 12.6.
 There is highly statistically significant difference (P<0.001) between pre and post as regard to Roland low back pain disability scale between groups. There is highly statistically significant difference (P<0.001) between pre and post as regard to pre Quebec low back pain disability scale in exercise group.
 There is highly statistically significant difference (P <0.001) between pre and post as regard post Quebec low back pain disability scale in acupressure group.
 There is a high statistically significant difference (P<0.001) between exercise group and acupressure group as regard Quebec low back pain disability scale.
Based on the findings of the present study, it is concluded that:
There is highly statistically significant improvement ”decrease in Quebec low back pain disability scale and decrease in Roland low back pain and disability scale” in (exercise group) post intervention.
There is highly statistically significant improvement ”decrease in Quebec low back pain disability scale and decrease in Roland low back pain and disability scale” in (acupressure group) post intervention.
Finally, acupressure therapy has more positive effect on reliving functional disabilities and low back pain among pregnant women than exercise. In the light of the findings of the study, the following points are recommended:
 Increase awareness of the pregnant women’s about low back pain and their treatment
 Educate nurses, pregnant women and support family members as husband or mother about proper technique of acupressure.
 Further researches for factors affecting application of acupressure therapy by nurses on antenatal clinic to relive low back pain among pregnant women.
