Abstract This is a retrospective study that include thirty-six patients with cytological / histological evidence of locally advanced non metastatic PAC, presented to clinical oncology department at Ain Shams university hospital during period from 1st of January 2011 till 31th of December 2016 and fulfilling the inclusion criteria, where some patients treated with chemotherapy alone compared to who were treated with concomitant chemo radiotherapy as a treatment for pancreatic cancer, obtained from filing system and had comparable characteristics. Regarding the ORR, CR in chemotherapy alone was higher, where the PR was more in concomitant chemo radiotherapy. PD during treatment occurred more in chemotherapy group than in chemo radio-therapy group, but progression occurred in all cases after a certain period of time. The median PFS for chemo radiotherapy was higher than in chemotherapy alone but OS was higher with chemotherapy than CCRT with non-significant differences between the two groups regarding survival rate. Regarding the hematological toxicity as anemia, neutropenia, thrombocytopenia and febrile neutropenia all were markedly higher in the chemotherapy group compared to chemo radiotherapy group. The GIT toxicity as diarrhea, mucositis, nausea, vomiting, all were higher in both groups but more prominent in chemo radiotherapy group. In conclusion, among patients with locally advanced pancreatic adenocarcinoma, there was no significant difference in overall survival and progression free survival with chemo radiotherapy compared with chemotherapy alone.
