Abstract

Severe acute malnutrition (SAM) is a global public health problem that results from sudden reductions in food intake or diet quality and is often combined with pathological causes
- It is defined as WHZ < −3 or MUAC < 115 millimeters, or the presence of bilateral pitting edema, or both .
- In this work, we tried to evaluate the myocardial function in children with severe acute malnutrition before and after nutritional rehabilitation This study was conducted on 30 children diagnosed with SAM according to WHO 2013, and compare with age ,sex matched control group . children with SAM were treated in the nutrition rehabilitation unit of pediatric department, Minia University according to WHO standardized protocol for management of severe malnutrition over the period from May 2016 to march 2017 .
- Both patients and control groups were subjected to full history taking, thorough clinical examination , laboratory investigations including (HB,TLC ,electrolytes), echo and tissue Doppler imaging.
- The most common complication associated with SAM were bronchopneumonia (23%) and dehydration(31.9%) and during the course of treatment one case died from septic shock .
- Our study revealed statically significant decrease in HB and TLC in patient groups than that of the control and decrease serum ionized calcium and serum potassium .
- Our study revealed high statically significant differences between the 2 groups regarding diastolic function pre nutritional rehabilitation as 40 % of malnourished children have diastolic dysfunction grade 1, 30 % grade 2 but 30% have normal diastolic function in comparison to control group while no statistical difference regarding systolic function between malnourished children and control group .
- Post nutritional rehabilitation anthropometric results , laboratory data improved significantly .
- There was improvement in diastolic function as 65 % have normal diastolic function in comparison to 30% pre rehabilitation
- Our study revealed significant positive correlation between diastolic function and post rehabilitation WAZ,WHZ,HAZ and MUAC parameters.
