Abstract Summary Myringoplasty is the definite treatment of tympanic membrane perforation. Our aim is complete closure with satisfactory hearing results. In our study we compared two groups each one is 25 patients with medium sized tympanic membrane perforation. In group “I”, we used platelet rich plasma enriched fat graft in myringoplasty and in group “II”, we used the classical cartilage perichondrium one. For the graft take rate, 22 (88%) patients had a successful outcome in the PRP enriched fat graft myringoplasty group and 23 (92 %) patients had a successful outcome in the classical cartilage perichondrium myringoplasty group. Regarding the postoperative hearing improvement, there was a significant improvement in both groups, in PRP enriched fat graft myringoplasty group “I”, the mean air bone gap was (20.76 ± 4.62) preoperatively and it was (6.64 ± 2.41) postoperatively. While in classical cartilage perichondrium myringoplasty group “II” the mean preoperative (A.B.G) was (22.76 ± 4.94) and postoperatively was (7.12 ± 2.71). These results showed significant postoperative hearing improvement, while there was no significant difference in the postoperative (A.B.G) between both groups. Finally, the PRP enriched fat graft myringoplasty is a minimally invasive, effective procedure in management of the medium sized tympanic membrane perforation. Also it is safe and feasible procedure, with good success rates for tympanic perforation closure and recovery of hearing thresholds and there was no significant difference in results between it and the classical cartilage perichondrium myringoplasty. Conclusion The PRP enriched fat graft myringoplasty is a minimally invasive, effective procedure in management of the medium sized tympanic membrane perforation.
