Abstract

C
omplexity of stone disease and advancement of technology increase the need to multiple new researches to understand and treat this disease.
Technical improvement and understanding of new tools of management of upper urinary tract stone disease increase success rate of retrograde intra renal surgery or ureteroscopy.
Ho: YAG laser lithotripsy is the most widely used laser nowadays in urology due to its safety. Ho: YAG laser is also used in other various urological diseases.
Laser settings is a new topic for researches nowadays, these researches are directed mainly to improve stone free rate and to decrease complications. The different techniques of Laser lithotripsy as dusting and basketing depends on Laser lithotripsy settings. Stone dusting settings are decreasing energy and increasing frequency resulting in converting a stone to dust or tiny fragments so that to allow these tiny fragments to pass spontaneously. Stone basketing is to convert the stone to visible fragments which can be extracted with forceps and baskets by increasing energy and decreasing frequency.
Our study was conducted at The Urology Department, El Maadi Military Hospital and Ain Shams University Hospitals starting from March 2017 to December 2017. Sixty patients were included in this study that aimed to compare between the outcome and complications of distal ureteric stone laser disintegration using dusting vs. basketing technique using the semirigid ureteroscope with focussing on stone free rate.
Our laser settings in this research was energy 0.5 J and frequency 20 Hz for dusting technique and 2 J energy and 5 Hz frequency for basketing technique. Stone free rates of dusting and basketing were 60.7% and 90% respectively. Post operative complications were 16.7% with dusting technique and 20% with basketing group.
