Abstract Background: Rhematogenous retinal detachment (RRD) is a serious, potentially sight threatening condition. If it does not promptly and effectively treated, it may leave the eye dysfunctional and atrophic. Retinal detachment occurs in 1 in 10,000 person/year. Successful repair of RRD has been reported since the 1930s, and numerous techniques have resulted in improving the surgical outcomes. Primary retinal detachment repair techniques include sclera buckling, pars planavitrectomy, and pneumatic retinopexy. Although all techniques show high re-attachment rates, each procedure has its own drawbacks and complications Objective: the aim of the study 1-primary outcome: to assess the effect of 360 laser retinopexy on prevention of recurrent retinal detachment after silicone oil removal 2-secandary outcome A-to assess the success rate of surgery B-to assess risk factors for recurrent retinal detachment Subjects and Methods:This study is a prospective randomized interventional study including 80 consecutive eyes suffering from primary RRD were treated in surgical retina department in research institute of ophthalmology all patients done by same surgeon with 23 G PPV, SiO injection and endolaser.Study period: from 1/1/2017 till 30/2/2018.Those 80 eyes were divided into two groups:Endolaser was applied to all exisiting breaks as well as 2-3 rows of 360 degree anterior to the equator in 40 patients.Endolaser was applied to the retinal breaks and did not receive 360 degree laser treatment in 40 patients.Patients included were those with primary RRDs with various grades of proliferative vitreoretinopathy (PVR) except for grade D. Results analysis: The comparison between two independent groups with qualitative data was done by using Chi-square test and/or Fisher exact test only when the expected count in any cell found less than 5.factors analysis done to identify statistically significant effect on the rates of re-detachment. The factors revealed in this analysis are 360° degree laser retinopexy (P value =0.039), preoperative visual acuity and the duration of retinal detachment (P value ≤ 0.01), PVR grade C (p value<0.001). Conclusion: In our study, the success rate for primary repair of RRDs was 75% after one surgery, and a final re-attachment rate of 92.5% after two surgeries and 360° laser treatment have statistical benefit in increasing the surgical success rates in retinal detachment repair with PVR grade C and duration more than 2 months. |
