Abstract

SNB using triple technique is considered as a standard technique in management of clinically negative neck for early stage HNSCC. The most standard technique is the triple one using LSG, gamma probe and blue dye. This technique has excellent detection rate and higher sensitivity and specificity in comparison to blue dye method. Triple technique has limited usage in Egypt due to limited resources; expensive radioactive materials. On the other hand, blue dye method is cheap but it is used in literatures mainly in presence of radioactive materials aiming for result augmentation by increasing rate of detection. Dye method is not considered till now as alternative for triple technique as absence of its result alone in long term follow up. In our study, we evaluated the probability and accuracy of SLNB using methylene blue dye in N0 neck staging in OSCC patients. from our study we conclude that: Methylene blue dye is a cheap, efficient alternative for patent blue dye with less allergic reactions. However triple technique (dye & isotope and gamma probe) still superior to a single method approach as regard both detection & false negative rates. SNB has no value in patients received radiotherapy mostly due to lymphatic obstruction. Blue dye method has limited value in prediction site of SN preoperative. So, formal neck incision and dissection are needed to localize SN. SNB must be utilized as an alternative for END in cN0 after finding applicable method according to our resources and prospective study as regard long term follow up on sentinel negative.
