Abstract

Introduction: Androgenetic alopecia is the result of progressive patterned hair loss that occurs when genetically predisposed individuals are exposed to androgens which occurs by androgen mediated conversion of terminal hairs to vellus hairs.Minoxidil and Finasteride are the only FDA approved treatment modalities for AGA. Microneedling is a novel and safe tool in the treatment of androgenetic alopecia.The aim of work: Is to evaluate the efficacy of microneedling technique (dermaroller) in male patients with androgenic alopecia under topical minoxidil 5% therapy. Research Plan:. This study was carried out on 40 male patients complaining of androgenetic alopecia collected from June 2015 to April 2016, between 20 years and 40 years of age with disease duration of less than 5 years with grade III vertex up to grade V AGA According to Norwood-Hamilton grading scale, digital photographs and dermoscopic images were taken before each treatment session, sixteen weeks treatment with topical minoxidil 5% 3 puffes on each side twice daily and eight treatment sessions with microneedling (dermaroller 1.5 mm) two weeks a part on right side. Assessment of the efficacy of therapeutic procedure by clinical assessment,patient self assessment and dermosopy assessment. Results&Conclusion: At the end of 16 weeks treatment period by clinical evaluation we found a non statistically significant difference in hair growth between two halves .However more cases with greatly increases of hair growth in microneedling treated half .There was statistically significant difference in patients satisfaction between two halves with very high satisfaction to their hair growth in microneedling treated half. Also we found statistically significant difference in mean terminal hair count between baseline and end of treatment between both halves of the scalp. in conclusion Pathogenesis of AGA is multifactorial and is still not clear,efficacy of minoxidil therapies with respect to new hair growth is unsatisfactory and the addition of microneedling procedure augments the response.
