Abstract

This prospective study was done in the duration between March and September 2016 and subjects were prospectively recruited from outpatient clinic of Minia Fever Hospital.
The aim of the study was to evaluate the efficacy of Diazo reagent as a diagnostic test of Salmonella infection in comparison to Widal test and blood culture in both cases and carriers of infection.
Subjects were grouped into three groups: group 1: 30 subjects were typhoid cases. group 2: 30 subjects which were carriers. group 3: 30 subjects which were normal volunteers.
The cases and carriers groups were in the same age group (33.7+/- 10.7 Vs 30+/- 7.5) with no significant difference, while the control group were older.
No gender difference in the diseased and control groups, while there was a male predominance in the carrier group.
No residence difference between the urban and rural areas as regard the three groups.
Workers were accounted for the majority of the diseased group while food handlers were the carrier group.

Clinically, hepatomegaly was not significantly common neither in the diseased nor in the carrier groups. On the other hand, splenomegaly was significantly common in those 2 groups.
As regard the laboratory findings, decreased platelet count was significantly higher in the diseased group in comparison to the other two groups.
Blood culture was only positive in 40% of the diseased group, while Widal was positive in 80% and Diazo 93%.
In the carrier group, Diazo test and Widal test were equally positive in 80% of cases while blood culture was negative in 100%.
As regard the accuracy, Diazo test had the highest accuracy with 98% with AUC 0.98, sensitivity 93% and specificity 100% in comparison to Widal test which had 80% accuracy, AUC 0.8, sensitivity 60% and 100% specificity.
As regard the diagnosis of carriers, Diazo test was equal to Widal with 90% accuracy and AUC 0.9, 80% sensitivity and 100% specificity.
