Abstract

Minor head injury accounts the most common type of head injury examined in emergency and accident departments, reaching approximately more than 85%of head injury cases No precise guidelines can confirm or exclude presence of intracranial pathology in cases of minor head trauma Numerous published guidelines which can help emergency and neurosurgery physician in predicting intracranial injuries such as Nice guidelines ,Canadian ,new Orleans and falmiriski rules This is a prospective study of 200 patients presented to Emergency Hospital, Mansoura University with polytrauma associated with evident minor head trauma. The patients were examined carefully ,investigated and evaluated according to the indicators of brain CT scanning in cases of minor head trauma among adult cases of minor head trauma among patients arrived to Mansoura University Emergency Hospital (MUEH) having all of the following: 1- more than 18 years old 2- History of blunt trauma to the head 3- GCS score of 13 or greater as determined by the treating physician; and injury within the past 24 h. They were collected and underwent the following steps : Adequate Resuscitation and careful 1ry survey followed by secondary survey then detailed Neurological examination was done After that Application of (Canadian, New Orleans ,Falmirski and NICE guidelines /rules)to all cases undergoing the present studywas done Next ,Radiological investigations was performed by NCCT brain for all cases undergoing the present study, uploaded on Ibn sina local health care registration system and interpreted with assistance of radiology and neurosurgery physicians Finally ;All data will be recorded and analyzed and the Outcome was estimated by: Abnormal CT brain finding reported for each case undergoing the present study The study Concluded : 1. Minor head injury accounts the most common type of head injury in Mansoura university emergency hospital 2. Nice guidelines represented the most reliable guidelines in predicting intracranial pathological finding in minor head trauma followed by Canadian guidelines 3. Major percentage of minor head trauma cases doesn’t need surgical treatment and most of them can be managed conservatively The study recommended : 1-Adoption of Nice guidelines for performing NCCT scan in minor head trauma in Mansoura university emergency hospital local trauma protocol to reduce economic costs and reduce unnecessary radiation exposure for cases of minor head trauma 2- Adequate education of emergency, neurosurgery physicians and radiology doctors and technicians about economic costs and radiation exposure health hazards 3-Health education for the community about economic costs, radiation exposure health hazards and catastrophic adverse effects of unnecessary imaging.
