Abstract

Hypertensive disorders during pregnancy represent the second commonest cause of direct maternal death and complicate an estimated 5–10 % of pregnancies.The cause involves inadequate cytotrophoblastic invasion of the myometrium, resulting in placental hypoperfusion and diffuse maternal endothelial dysfunction. Changes in angiogenic and antiangiogentic peptide profiles precede the onset of clinical preeclampsia.
Women with preeclampsia should be closely monitored and receive magnesium sulfate intravenously if severe features, HELLP syndrome, or eclampsia occur & definitive therapy is delivery of the fetus.Hypertension in pregnancy increases future maternal risk of hypertension and cardiovascular disorders.
The present study reveals all patients with hypertension during pregnancy that had been managed at Minia University Hospital from January 2016 to December 2016. This Hospital is a population-based facility, which provides primary, secondry and tertiary care for women of Al-Minia Governorate.
During the twelve months study period total 12331 women were delivered, out of them 721 were diagnosed as cases of hypertensive disorders during pregnancy.
The aim of this study was to determine the frequency and distribution of different types of hypertensive disorders of pregnancy, and to determine the impact of this potentially devastating disorder on maternal and fetal outcome at Al-Minia University Hospital, which is a population-based facility that provides primary, secondary and tertiary care to women of Al-Minia Governorate.
We found in The present study Gestational hypertension represents 29.1% of all cases , Mild preeclampsia 23.5% , Severe preeclampsia 19.7% , Imminent eclampsia 15.1% , antepartum eclampsia 8.2% , postpartum eclampsia 7.4% , HELLP 7.9% , Renal failure 3.1% , Hepatic failure 4% , Pulmonary edema 2% and maternal mortality represents 1% of all cases .
Also we found Preterm 36.4% of newlyborn , Miscarriage 3.3% , IUFD 10.7% IUGR 20.5%, early neonatal death 4.2% and NICU admission was 26.5% of babies .
As a conclusion, the frequency of hypertensive disorders of pregnancy is high in this study.The disorder is associated with high maternal and perinatal morbidity and mortality. The devastating effects of the condition could be prevented with close antenatal follow up.
Further prospective studies from different areas in the country are needed to establish the frequency, clinical pattern and impact of this disorder to guide health care providers.
