Abstract

Summary : Psychiatric consultation liaison nurses, or psychiatric intake nurses, work at acute psychiatric care facilities, hospital emergency rooms and mental health clinics. They evaluate the mental health of patients, recommend treatment and provide care options and participate in discharge planning (Henderson, 2015). Broadly, a consultation liaison (CL) nurse focuses on the mental health care of people in non-psychiatric (generalist) settings, most commonly but not exclusively, in general hospital wards, emergency departments, and nursing homes. The CL nurse aims to improve outcomes in patient care from a mental health perspective through working directly with patients and indirectly through increasing the capacity of staff to recognize and attend to the mental health needs of patients in these settings (McNamara, 2014). Epilepsy is a chronic neurological condition of unprovoked, recurrent seizures. The disorder affects approximately 1% of the population, including over one million women of childbearing age. Women with epilepsy face additional challenges secondary to hormonal influences on seizure activity and endocrine function. In addition, antiepileptic drugs (AEDs) play a significant role regarding contraception, pregnancy/breastfeeding and bone health (Sunila et al, 2009). The unpredictable nature of epilepsy can cause distress in persons even with well-controlled seizures and result in negative consequences, and this distress is usually involving loss of independence, negative view of one’s ability or efficacy, and dealing with embarrassment and stigma associated with seizures. Aim of the study: To design, assess, implement and evaluate the effectiveness of psychiatric consultation liaison nursing program on psychological stressors and self- efficacy among epileptic women.Research Hypothesis:H1- Epileptic patients who exposed to psychiatric consultation liaison nursing program will have higher scores in self- efficacy in the post test program more than the pre test scores.H2- Epileptic patients who exposed to psychiatric consultation liaison nursing program will have lower scores in stressors in the post test program less than the pre test scores.Setting of the study:The study was conducted at the outpatient clinic of epilepsy at Assiut University Hospital. This clinic is working only two day every week (Monday and Thursday), average number of patients’ frequency weekly approximately 50 patients. This setting is serving 4 Governorates (Assiut,Sohag, Qena and Aswan) . This hospital provides all necessary health services free of charge.Subjects: A sample of convenience includes (30) women who had been diagnosed as epileptic patients and agreed to participate in the study and was selected according to set criteria.The data were collected by using the following tools:-1- Demographic and clinical data sheet (Appendix I).2- General Self Efficacy Questionnaire (GSEQ) (Appendix 2).3- Epilepsy Self Efficacy Scale (ESES) (Appendix 3).4- Stressors Inventory for Persons with Epilepsy (SIPE) (Appendix 4).Consultation liaison nursing program:The program development:-The program was designed by the researcher and revised by supervisors. The program included two parts. The first part was implemented to inform the women with epilepsy disorder about definition, causes, types, signs and symptoms of epilepsy as well as nursing care of this disorder, while the second part was implemented to improve coping skills of those women by teaching them relaxation techniques. The program consisted of 12 sessions, one session per week for an hour. A pilot study was carried out to ascertain the clarity and applicability of the tools. Data were collected in 10 months period from February 2015 to November 2015. Data obtained were presented in number and percentages, X2, p value, paired t test were used to compare these percentages.The main results of the present study:-More than half of the studied sample (56.7%) was observed in the age group ranged from 30-40 years , there were no statistically significant differences for both single and married women as regards marital status, 90% of the studied group were from rural area, and 96.7% of the studied group were housewife, and more than half of them were illiterate, read & write (56.7%). More than one third of the study sample was with duration of illness >15 years, relating to frequency of fits, ”every few months” represented the highest score (73.3%).Improvement of knowledge and practice related to relaxation techniques of epileptic women after application of the consultation liaison nursing program.There were significant differences between pre, post program, and follow up regarding to all subscales of epilepsy self-efficacy scale.There were significant differences between pre, post program, and follow up in all items of Stressors Inventory for Persons with Epilepsy of epileptic women.There were significant negative correlation between levels of self-efficacy and stressors in pre, post program, and follow up in epileptic women.Based on the results of the study, it was recommended that, Community services should be offered to women with epilepsy as counseling center helps them manage their seizures and their lives
