Abstract

Nurses need to ensure they provide a high standard of hemodialysis practice by delivering safe and effective patient care in a supportive and comfortable environment. Their poor performance will affect the level of quality of care given to the patient receiving hemodialysis. There must be a clear identification of the skills and knowledge required by the nurse in order to carry out patient care effectively. Therefore, there is a need to provide continuous education to the nurses working in a dialysis unit for hemodialysis nursing practices (Hassona, 2011):
The present study has been designed to assess nurses’ performance regarding patient undergoing hemodialysis.
This aim will be achieved through the following:
1-Assessing staff nurses’ knowledge regarding hem dialysis patients.
2-Assessing nursing ’ practice regarding hem dialysis.
Research question:
The current study answered two questions
1-What are the nurses’ knowledge regarding hemodialysis patient?
2-What are the nurses’ practices regarding hemodialysis patient?
Research design:
A descriptive design was utilized for the conduction of this study.
Setting:
The present study was conducted at hemo dialysis unit EL Fayuom General Hospitals
Subject:
The subject included all available nurses as a convenience sample, including 30 nurses after obtaining their consent to participate in the study.
Tools used to accomplish the aim of this study:
Two tools were used in the current study as follows
1- Interview Questionnaire sheet (appendix 1): it will be cover data related to nurses’ knowledge who work with hemodialysis patients (pre hemodialysis – during –post) etc. developing by the research to Assess nurse Knowledge regarding patient hemodialysis.
2- An observational checklist (appendix 2): it will cover data related to nurse’s practices who work hemodialysis patients. To observe infection control checklist – drug administration patient undergoing hemodialysis checklist and program machine checklist.
Results:
The results of the study showed that:
• The mean age for nurses included in the study was (37.15±6.32 (, while all of the study nurses were males &females. Also it reveal that; the majority of them were females more than (50.0%) of them had technical nursing More than one third (73.3%) of nurses under the study had less than 5 years of experience & (53.3%)didn’t attend any nursing training course.
• About thirds (36.3%) of them their total knowledge regarding to hemodialysis assessment were unsatisfactory.
• About slight less than half (43.3%) of them their total practice regarding to hemodialysis assessment were unsatisfactory.
• More than two thirds (56.7%) of them were male and their total knowledge regarding to hemodialysis assessment were satisfactory.
• There was statistical significant difference between nurses’ knowledge and their age with mean10.048 and p value 0.018
• There was statistical significant difference between nurses’ knowledge and level of education with mean 7.081and P-value=0.025.
• There was statistical significant difference between nurses’ knowledge and experience with mean 3.282and P-value=0.194.
• There was statistical significant difference between nurses’ knowledge and attendance of training course with mean 0.010and P-value=0.919.
• There was statistical significant difference between nurses’ practice and age with mean 8.824and P-value=0.032.
• There was statistical significant difference between nurses’ practice and educational level with mean 11.041and P-value=0.002.
• There was statistical significant difference between nurses’ practice and experience with mean 4.733and P-value=0.094.
• There was statistical significant difference between total knowledge and their total practice.
Conclusion
Based on the findings of the present study, it can be concluded that:
About one quarter of studied nurses had unsatisfactory knowledge and less than half of them had unsatisfactory practice regarding hemodialysis assessment. the studied showed statistically significant correlation between nurses’ knowledge and nurses’ practice regarding hemodialysis assessment and their demographic characteristics as regards: Age, gender education, experience &training course.
Recommendations
Based on the results of the present study, the following recommendations are suggested:
• Develop an orientation program for newly appointed staff nurses in hemodialysis
• Develop health teaching programs, nephrology staff for hemodialysis patient, through a simple booklet (related to disease, treatment, diet practice related to hemodialysis) with update knowledge and instruction about chronic renal failure.
• The study should be replicated on large sample and in different hospitals setting in order to generalize the results.
• Close supervision and teaching on spot is needed to ensure that quality of care is provided by nurses while assessing hemodialysis patients.
• Increase the number of nurses in each dialysis unit based on international nurse patient ratio to improve quality of care.
• Importance of implementing educational training program for nurses in hemodialysis unit regarding hemodialysis assessment.
