Abstract

Acute respiratory failure remains a major cause of morbidity and mortality in the intensive care unit providing best possible patient care is the most important goal of any health care facility. In assessing the patient, the nurse evaluates the patient’s physiologic status and how the patient is coping with mechanical ventilation. The nurse should assess the patient’s comfort level and ability to communicate as well moreover, there are many factors affect on nurses work.
Aim of the study:
The study aims to assess factors affecting nurses’ performance for patients with acute respiratory failure on mechanical ventilator through the following:
1-Assess nurses’ performance (level of knowledge, practice, and attitude) regarding patients with acute respiratory failure on mechanical ventilator.
2-Identify the factors affecting nurses’ performance for patients with acute respiratory failure on mechanical ventilator.
Research questions:
-What are the nurses’ level of knowledge, practice and attitude regarding patients with acute respiratory failure on mechanical ventilator?
-What are the factors affecting nurses’ performance in caring for patients with acute respiratory failure on mechanical ventilator?
Subject and Methods:
Research design:
A descriptive exploratory design will be used in carrying out this study.
• Setting:
This study will be conducted at chest intensive care unit in Bani – Suief University Hospital.
• Subjects:
A purposive study subject of 30 nurses’ caring for patients with acute respiratory failure on mechanical ventilator in the previously mentioned setting for not less than 6 months duration will be recruited to this study.
• Tools of data collection:
Data will be collected using the following tools:
1-Nurses’ demographic assessment self administered questionnaire
It was used to assess nurses’ demographic characteristic of the studied nurses as regarding age, gender, level of education, years of experience, training courses.
2-Nurses’ knowledge assessment self administered questionnaire
It was used to assess nurses’ level of knowledge regarding caring of patients with acute respiratory failure on mechanical ventilator.
3-Nurses’ practice observational checklist
It was used to assess nurses’ level of practice regarding caring of patients with acute respiratory failure on mechanical ventilator.
4-Nurses’ attitude likert scale
It was used to assess nurses’ level of attitude regarding caring of patients with acute respiratory failure on mechanical ventilation.
5-Factors affecting nursing performance assessment questionnaire
It was used to assess factors affecting nurses’ performance during providing care for the patients with acute respiratory failure on mechanical ventilator.
The present study revealed that:
Regarding nurses’ demographic characteristics 96.7%, of the study nurses’ ages were between 20 to <35 years and 70.0% had experience less than 5 years. In addition, 83.8% of them were females, 53.3% of them were Bachelor degree in nursing and 60.0% of them no attended training courses.
Concerning the study nurses regarding caring patients with acute respiratory failure on mechanical ventilation, 57% of them had unsatisfactory total level of knowledge, 50% of them had satisfactory total level of practice while, 57% of them had satisfactory level of attitude.
However, 100% of them stated major physical factor regarding suffer from physically burdens from staff shortage, 96.7% of them stated major social stressor regarding annoyed to participate socially especially on official occasions also, 93.3% of them stated major psychological factor regarding frustrated when a patient does not improve and 100% of them stated major administrative factor regarding fewest nursing professional in the units. Furthermore there was no statistical significant correlation between studied nurses’ total level knowledge, practice and attitude.
Also, there was no significant correlation between total level of the nurses’ knowledge and any other factors. Total practice has a high statistically significant negative correlation with psychological factor. While, total attitude has a statistically significant negative correlation with social pressure factor.
Conclusion
The study revealed that more than half of studied nurses had unsatisfactory total level of knowledge and attitude, half of them had satisfactory total level of practice. Moreover, all most of them stated major physical factor regarding suffer from physically burdens from staff shortage and social stressors regarding annoyed to participate socially especially on official occasions. Also, majority of them stated major psychological factor regarding frustrated when a patient does not improve and administrative factors regarding fewest nursing professional in the units. Additionally to, there was no statistically significant correlation between the studied nurses’ total level knowledge, practice, attitude. Also, there was no significant correlation between total level of the nurses’ knowledge and any other factors. Total practice has a high statistically significant negative correlation with psychological factor. While, total attitude has a statistically significant negative correlation with social pressure factor.
Recommendations
• Replication of the current study on large sample and different hospitals settings to be able to generalize the results.
• Educational training program for nurses should be designed to improve nurses’ practice in chest care units regarding caring patients with acute respiratory failure on mechanical ventilation.
• Chest care units should be supplied by a protocol of nursing care for patients with acute respiratory failure on mechanical ventilation.
• A simplified and comprehensive booklet should be designed including guidelines about caring for patients with acute respiratory failure on mechanical ventilation.
