Abstract

Background: mechanical thrombectomy is gaining favor worldwide in the combat against acute ischemic stroke. This has been supported by many randomized controlled trials. The stent retrievers and primary aspiration represent the two main thrombectomy techniques nowadays. In this study we compare the results of the two techniques; clinically and radiologically. Patients and methods: we have retrospectively analyzed our prospectively prepared database to compare the data of 2 equal groups of consecutive patients; with 35 patients in each group. One of the groups received stent retriever thrombectomy as a first line treatment for acute ischemic stroke, while the other group received primary aspiration. The examined parameters included the perioperative details, the degree of recanalization, and the clinical outcome. Results: The primary aspiration group had significantly less operative time, less number of passes, and less iatrogenic emboli. The recanalization rate with aspiration was 97.1%, while it was only 77% with stent retrievers. The stent retriever group had better clinical outcome and less mortality. Conclusions: The primary aspiration technique is a safe, cheap and effective technique that needs to be further evaluated using randomized controlled trials.
