Abstract

Type 1 diabetes mellitus (T1DM) is the most common metabolic disease among children, adolescents and young adults and its incidence rate is still rapidly increasing. As the onset of the disease occurs in early life, the afflicted are at great risk of developing cardiovascular disease as a complication of diabetes. The majority of ischemic events - occur due to intravascular thrombosis. This is a state which favour platelet aggregation and adhesion . Mean platelet volume (MPV) and platelet distribution width (PDW) are morphometric indices reflecting the size of distribution of the peripheral platelet population.
Aim of the study:
The aim of our study was to investigate whether platelets’ morphology is altered in children with Type 1 Diabetes, and its relation to disease duration and metabolic control of diabetes mellitus in those children.
Subjects and methods:
This case-control study included 75 children who were already diagnosed as having diabetes mellitus and had regular follow up visits at the diabetes out-patients’ clinic or were admitted in the diabetes unit at Minia University children’s hospital. They were divided into three groups. group I; included 25 children recently diagnosed with T1D as regard ADA, 2014 within 1st 6 months of diagnosis. group II; included 25 children with T1D for duration (1-5) years. group III; Included 25 children with T1D with duration more than 5 years. Another, 25 apparently healthy children group IV , age and sex matched with the diabetic children served as a control group.
The study was performed during the period from December 2015 to August 2016.
The studied groups were subjected to the following :Thorough History taking ,clinical examination and Laboratory investigations included: Random blood sugar, Complete blood count including: Hemoglobin ,Total leucocytic count ,Platelets(platelet count ,mean platelet volume, platelet distribution width, platelet large cell ratio,
Results:
As regard the results of the current study, we found that
The current study found that the mean age of diabetic children was12.2 ± 3, most of them were females 60% versus 40% males, 64% from rural areas with 36% from urban.
Concerning platelet morphological indices(MPV,PDW P-LCR), the diabetic group had significantly higher levels than the control group where (p<0.001).BMI had significant fair correlation with PDW and P-LCR(r=0.268&p=0.020,r=0.261&p=0.024).Duration of diabetes had significant positive moderate correlation with MPV(r=0.630&p=0.001) and significant moderate positive correlation with P-LCR(r=0.507&p=0.001).Fructoseamine had moderate significant positive correlation with MPV(r=0.612&p=.001) and significant fair positive correlation with PDWC and P-LCR PDW (r=0.459 & p = 0.001) ,(r=0.347&p=0.002).
