Abstract

Background: SLE is a heterogeneous autoimmune disease that affects virtually all organ systems with varying degrees of severity, Infection,cardiovascular disease, and organ failure are among the leading causes of death in almost all the studies looking at mortality in SLE. Objectives: In our study, we aimed to determine the prevalence of critical complications of SLE &mortality rate to analyze complications risk factors in adults with SLE. Methods: fifty SLE patients were included in this a retrospective&prospective study .Their medical files were retrospectively reviewed for the personal history, complaint, present history, present medication, past history, family history, clinical features of the disease, immune profile, laboratory features and disease activity (SLEDAI).Results: In our study females represent 84%, as regards to age the mean was (29.3±8.7) years old ranged between 13 and 52 years old. Mortality was 24% .The commonest cause of death was infection .The second common cause of death was pulmonary complications, metabolic acidosis, cardiac complications, neurological complications and renal failure. In our study we demonstrated that SLE disease activity index among non survivors had mean 41.8 &p-value 0.001 , we noticed that sever
SLE activity was a high risk predictor for mortality with p-value <0.001 &
positive correlation with mortality with p-value <0.001.In our study SLE index sensitivity (100%) and specificity (73.7%) at cut off value (28.5).Conclusion: SLE is a rather not uncommon disease in Fayoum governorate .The late presentation of patients may explain the large incidence of organ damage mandating referral to the ICU.
