Abstract

This study was conducted on 47 patients presenting with various symptoms include pelvic pain and abnormal uterine bleeding.
The aim of this work is to study the role and value of trans-vaginal ultrasound (TVUS) and Doppler as a method to diagnose and differentiate between adnexal masses.
All patients were subjected to full history taking and clinical examination and transvaginal ultrasonography of the pelvis GE Logic-P6 US, GE Logic-C5 and Hitachi Hi Vision machines.
The results are concluded as follow:
• This study included 47 female. Their age ranged between 22 years old and 66 years old. The mean age for the benign cases was 34.79 and the mean age for the malignant cases was 57.8.
• Based on the Ultrasound (US) findings the consistency of the masses was cystic in 25 cases, complex cystic with solid component in 12 cases and solid in 10 cases. All the cystic masses were benign while among the complex cystic with solid component cases 9 was benign and 3 was malignant. The solid cases show 8 benign and 2 malignant.
• By Doppler and colored duplex studies we noted vascularity in 23 cases (48.5%) and 24 case (51.1%) were avascular. Among vascular masses, 18 case were benign (42.9%) and 5 cases were malignant (100%).
• Vascular indices were calculated for the masses with vascularity RI and PI, revealing a high diagnostic value in predicting malignancy in various adnexal masses.
from the previous results and discussion we can conclude that: even though histo-pathological examination of the adnexal lesion is the gold standard for diagnosis or exclusion of malignancy, 2D transvaginal U/S in addition to Doppler are reasonably accurate, helpful and non-invasive tool for assessing adnexal lesions.
So, we conclude in this study that the transvaginal US with Doppler is a useful tool in the diagnosis and differentiation of adnexal masses.
