Abstract

The current study was acase control study conducted on 100 patients with Type 2 diabetes mellitus (DM) and was divided into two groups first is 50 diabetic patientswith metabolic syndrome and 50 diabetic patients without metabolic syndrome.Serum magnesium levels were done for all patients to search about the relation between serum magnesium and different components of metabolic syndrome in both groups. The metabolic syndrome was diagnosed in the presence of at least two of the followings as regard all patients known to have type 2 diabetes, elevated blood pressure (≥130/85), abdominal obesity WC (for men ≥102 cm,for women ≥88 cm), hypertriglyceridaemia (TG ≥ 150mg/dl). Low HDL (for men < 40mg/dl , for women < 50 mg/dl).Hypomagnesemia is defined as a serum Mg concentration < 1.8 mg/dL (normal range: 1.8 mg/dL – 2.3 mg/dL).In our study there was a significant decrease in serum magnesium level in type 2 DM patients as compared to control group. Our study has shown a significant negative correlation between magnesium and fasting plasma glucose, post prandial plasma glucose in patients with type 2 diabetes mellitus especially if associated with metabolic syndrome. In our study there was a significant negative correlation between magnesium and HbA1C in diabetic patient with or without metabolic syndrome.
The current study showed that the hypomagnesaemia is more frequent in diabetic patient with metabolic syndrome (23 patients from 50 patients (46%) in diabetic with MetS group compared to 19 from 50 patients (38%) in diabetic patient without MetS group). In our study there was a significant negative correlation between serum magnesium and serum triglycerides in diabetic patient with or without metabolic syndrome. In our study there was a significant decrease in serum HDL levels (p<0.00) observed in diabetic patient with or without metabolic syndrome when compared to control group, and a significant positive corrrelation between serum magnesium and serum HDL levels in diabetics with metabolic syndrome.
In conclusion, this study showed that the hypomagnesaemia is associated with type 2 diabetes especially uncontrolled patients .The presence of metabolic syndrome increases risk of hypomagnesaemia among type 2 diabetic patients.
Recommendations
Based on the results of this study, we recommend:
1-Regular follow up of type 2 diabetic patients for serum magnesium level.
1-All type 2 diabetic patients should be screened carefully for the presence of metabolic syndrome as this increase the risk of hypomagnesaemia. 3- the diet of type 2 diabetic patients should include magnesium rich foods such as whole grains, beans, nuts, and green leafy vegetables to guard against hypomagnesaemia.
