Abstract

Objective: aims to compare the efficacy of LNG - IUD (Levonogestral releasing intrauterine device) Versus Low dose COCs (combined oral contraceptive pills) on management of heavy menstrual bleeding and dysmenorrhea caused by adenomyosisDesign:Randomised control study ,Setting: Gynecology clinic of Ain shams university hospital.Patient(s): included 32 women with adenomyosis, Intervention(s): they were divided into two groups:group A: include 16 women who received Levonogestral releasing intrauterine device (Metraplant E).group B: include 16 women who received combined oral contraceptive pills (Yasmin)Main Outcome Measure(s);This study aims to compare the efficacy of LNG - IUD (Levonogestral releasing intrauterine device) Versus Low dose COCs (combined oral contraceptive pills) on management of heavy menstrual bleeding and dysmenorrhea caused by adenomyosis Result(s): The current study showed that after 6 months of treatment both groups improved regarding decrease in severity of dysmenorrhea score and heaviness of menstruation score, however decrease in severity of dysmenorrhea score was significantly remarkable in LNG group more than that COCs group (50% (40.2 to 83.3) versus 35.4%(28.6 to 47.2), p=0.004) and decrease in menstruation score was 29.4% (22.1 to 36.1) in group A versus 19.1%(13.7 to 26.8) in group B respectively. Conclusion(s): Levenogestrel releasing intrauterine device and continuous combined contraceptive pills are both effective in treating pain and heavy menstrual bleeding related adenomyosis after 6 months of treatment. However, LNG-IUD is better than COCs for relief dysmenorrhea but has the same effect regarding heavy menstrual bleeding and hemoglobin level
