Abstract The nonoperative management is considered the ideal management for blunt splenic injuries in pediatrics due to less complication ,less blood transfusion ,less hospital stay and less mortality than operative management. Criteria for nonoperative management: 1-Hemodynamic stability. 2- No signs of peritonitis . 3-No other associated injuries indicate laparotomy. 4- CT for documentation and grading of injury. 5-Specialized center with ICU and operative team available 24 hour. The operative management is indicated in 1- Hemodynamic instability. 2- Evidence of peritonitis. 3-Presence of other associated injuries indicate laparotomy. 4- Failure of NOM |
