Abstract

Reduction mammoplasty is a clear example of interface between aesthetic and reconstructive surgery. Improved understanding of anatomy and continuous evolution in surgical techniques helped to shift the focus from simple reduction surgery to the aim of keeping longer lasting results with maintained shape, function and sensation with less obvious scars. A good reduction mammoplasty should achieve adequate removal of excess breast tissue, preserve the nipple areola complex vasculature and sensation, gives long lasting aesthetic results, acceptable scars and keeps the ability of nursing post operatively.
Although the breast septum was described since 1998 by Wuringer et al; as a fixed anatomical structure carrying the main nipple areola complex neurovascular supply, there were no much studies which described the mammoplasty techniques based on this structure.
The aim of this study is to evaluate and elicit our experience regarding this reduction technique based on the breast septum.
30 cases suffering from macromastia were operated upon using the septum based breast reduction either lateral or medial pedicle, the pedicle selection either (medial or lateral) depended on the amount of excess tissue, age of the patient, position of the nipple and the patient medical conditions.
Patients and methods:
Detailed preoperative clinical assessment, general and local examination, routine preoperative laboratory investigations, the surgical procedures were determined medial or lateral septum based reduction according to each patient, The operative data were collected and documented, pre and post-operative photos for every case were taken in the designated periods. Every case received a discharge summary included date of surgery, hospital stay, the condition of nipple areola complex sensation and vasculature, any recorded complications if occurred and the follow up date, all the patients were followed up in the outpatient clinic 10 days, 2 weeks, 1 month, 3 months, and 6 moths post operatively.
