Abstract

TQM is a system that makes quality the responsibility of all clinicians, nursing personnel, and administrators throughout the health care organization. The TQM systems are established to prevent clinical and administrative problems, increase patient satisfaction, continuously improve the organization’s processes, and provide healthcare services as good as or better than of the competitors. TQM encourages continuing staff nurses’ education to ensure nursing staff are equipped with sufficient competency and knowledge to respond to the patient’s nursing care needs.The unit nurse manager is a significant facilitator of quality management and the incorporation of process improvement into day-today management functions. The success of a TQM program demands that unit nurse manager be involved by providing leadership to the nursing staff. They need to embrace their accountability for quality issues as envisioned in their quality focus.This study was carried to evaluate TQM educational program for unit nurse managers at Menoufia University hospital. This was achieved
through:▪ Assessing need for TQM practices from unit nurse manager, staff nurses and patients and their families’ perspectives.▪ Assess unit nurse managers’ knowledge on TQM.▪ Developing and implementing educational program of TQM for unit nurse managers.▪ Evaluating unit nurse managers change in level of knowledge on
TQM post program and at follow-up from pre-program.The study was conducted in Menoufia University hospitals which composed of three hospitals (Main hospital, Specialty hospital, and Emergency hospital) and Oncology institute.Three groups of subjects were included in this study which are: all(60) unit nurse managers, staff nurses (280) and simple random sample(500) of patients and their family from Menoufia University hospitals.Three questionnaires were used for data collection to assess TQM
practices from unit nurse managers, staff nurses, and patients and
families’ perspective.▪ Unit nurse managers’ knowledge test (pre, post, and follow-up).▪ TQM program included six sessions concerning (TQM in healthcare, organization development, organizational effectiveness,evaluation of patient outcomes, nursing informatic system, and patient right, responsibilities & nurse manager role) was developed,implemented and evaluated by researcher.Main result of study revealed the following:▪ Unit nurse managers (33.3%) were working in the Main University
hospital, (83.3%) had bachelor degree, (51.66%) were in the age group 35- < 40 years, (46.8%) had 10-< 14 years of experience,and (88.3%) of them not attended any previous quality programs.▪ Staff nurses (35.7%) were working in the Specialty hospital, and(57.1%) had nursing diplom, (44.28%) had 1-<10years of experience and (39.3%) of staff nurses were in the age group 25- <35 years.▪ Equal percent (30%) of patients was in each of the Main University hospital and Specialty hospital, (36.4%) had prep& secondary school education and (48.4%) were at age group 25- < 50 years.▪ Above half of unit nurse managers (52%) their perception of TQM practices was at poor level.▪ Below half of staff nurses (45.4%) their perception of TQM practices was at good level.▪ Above half of patients and their families (59.5%) their perception of TQM practices was at good level▪ There was negative correlation regarding TQM practice level among study subject.▪ There was negative correlation regarding to organizational value and scorecard excellence criteria among the study subjects.Program results▪ The majority (61.7%) of unit nurse managers’ total knowledge of
TQM program were at poor level pre-program, while (98.3% and93.3%) of them their total knowledge of TQM program was atgood level for post-test and follow-up test.▪ Half (50%) of unit nurse managers at Main university hospital their total knowledge of TQM program were at poor level pre-program,
while (100% and 95%) of them their total knowledge of TQM
program were at good level for post-test and follow-up test.
▪ The majority (81.3%) of unit nurse managers at Specialty hospital
their total knowledge of TQM program was at poor level preprogram, while (100% and 87.5%) of them their total knowledge of
TQM program was at good level for post-test and follow-up test.
▪ Above half (56%) of unit nurse managers at Emergency hospital
their total knowledge of TQM program was at poor level preprogram,
while (94% and 100%) of them their total knowledge of
TQM program was at good level for post-test and follow-up test.
▪ The majority (66.7%) of unit nurse managers at Oncology institute
their total knowledge of TQM program was at poor level preprogram,
while all of them their total knowledge of TQM program
was at good level for post-test and follow-up test.
▪ Half and above of nurse managers ranged (50% - 81%) in all
hospitals their total knowledge of TQM was at poor level preprogram,
while majority of them ranged (94.4% - 100%) their total knowledge of TQM was at good level for post and follow-up tests with statistical significant differences among hospitals at p-value ≤0.05.The proposed recommendation was as follows: -▪ Prime importance the concept of TQM be emphasized in undergraduate and post graduate curricula to build students basic knowledge on TQM.Each hospital recommended to▪ Provide nursing personnel with opportunities for promotion and growth to be satisfied and committed to their nursing roles for hospital improvement.▪ Establish training center to periodically educate nursing staff to provide patient with high quality, safe and efficient care.▪ TQM culture must be introduced and maintained to all healthcare staff to meet quality standard.▪ Patient perception and satisfaction should be measured and
considered as a tool for developing quality of care.Unit nurse managers (nursing supervisors) at each hospital under
study recommended to: -
▪ Develop, publish and advertise TQM framework include value,
objective, and quality guidelines.
▪ Be role model of TQM culture for staff nurses.
▪ Develop clear and effective strategies and supporting plans for
achieving TQM objectives.
▪ Reviewing and be familiar with process, methods and policies to
improve unit structure, operations and outcome.
▪ Motivating and coaching staff nurses and employees in her unit to define and achieve objectives of TQM.▪ Provide leadership to unit staff nurses and equip them with the tools to perform knowledge.▪ Establish TQM committee to control, direct and make resources allocation for the unit.Recommendation for future research:▪ Investigate TQM contributing and facilitating factors and barriers.▪ Study the role of unit nurse manager in creating a quality culture.▪ Study unit nurse manager level for adopting continual change.
