Abstract

The present study includes 27 young adult patients (30 hips); who underwent cementless metal on polyethylene THR in the Beni Suef University Hospital. Their ages ranged between (25-30y) with a mean of (28.4y), the follow up range (6-24 months).
24 patients had done unilateral THR and three patients had done bilateral THR. 19 patients were male (70.4%), while only eight patients were female (29.6%). The study include 14 right hips (46.7%) and 16 left hips (53.3%). Three cases were bilateral, one of them was male suffering from rheumatoid and the other 2 patient were suffering from advanced avascular necrosis.
The main indication for surgery in this study was advanced avascular necrosis (17 hips) (56.7%), Rheumatoid arthritis (eight hips) (23.3%), Traumatic (three hips) (10%) and pediatric pathology (three hips) (10%).
All cases were subjected to full clinical examination, roentegongraphic examination (in form of X-ray pelvis and X-ray of affected hip lateral view as well as X-ray of the whole affected femur side) and laboratory evaluation before they transferred to operation theatre for undergoing cementless THR the clinical assessment of the cases depends on HHS with total score (100) in which:
Below (70) considered poor.
from (70-79) considered fair.
from (80-89) considered good.
from (90-100) considered excellent.
The mean preoperative HHS was 17.5±12.9, while the mean of the postoperative score was 84.4±12.7. The difference is statistically highly significant and The mean preoperative pain score was 8±6.1, while postoperatively was 39.8±5.5 (there was marked improvement postoperatively and the difference was highly statistically significant as 11 hips (36.7%) had no pain postoperatively, 15 hip (50 %) had slight occasional pain while three hips (10%) had Mild pain with no effect on average activities while one hip (3.33%) had moderate pain which cause limitation of usual activities.
This shows that the cementless total hip replacement was effective procedure to relieve the pain and improve the life quality of the young active patients in the study. And that Total hip replacement is one of the most successful replacement operations in orthopedic surgery for its major effect in relieving pain and disability. With the increase of young active patients doing the surgery many designs were implemented and used to overcome the drawbacks associated with traditional total hip in young patients.
The complications recorded in this study were as follow: superficial infection in one hip (3.33%), calcar crack in four hips (13.4%), dislocation of one hip representing (3.33%), leg length discrepancy was reported in four hips (13.33%), aseptic loosening of acetabular components in two hips (6.66%), while revision was done to one case (3.33%).
In spite of the encouraging results in our study on the total hip replacement in young active patients in the short term follow up further evaluation is recommended to evaluate the results on the long run.
