Abstract

In a study that tested WM integrity, and examining its relationship to treatment response, DTI data were acquired in patients with first episode psychosis. Response was assessed after 12weeks and patients were classified as responders or non-responders according to treatment outcome. They also underwent a second diffusion tensor-imaging scan. At baseline, non-responders showed lower fractional anisotropy than both responders. After 12 weeks, there was an increase in fractional anisotropy in both responders and non-responders, positively correlated with antipsychotic exposure. These data, together with earlier findings on cortical grey matter, suggest that grey and white matter integrity is an important moderator of response to antipsychotics. Such pervious similar Studies, suggested an association between less extensive fractional anisotropy reductions and subsequent good clinical outcome, in the whole brain.
Aim of the work: To demonstrate difference in the presentation of symptoms, course and recovery between adolescents and adults with first episode psychosis at acute stage; To study the differences in neuroimaging and white matter affection between adolescents and adults with first episode psychosis acute stage; To identify different cognitive functional impairment among adults and adolescents with first episode psychosis acute stage; To compare the outcome measures with the different clinical variables in both groups; To find the independent risk factors that might be associated with the outcome.
This study is one of the early studies aiming to find out the outcome variables of the first episode psychosis (FEP)in the Egyptian population.FEP was studiedduring its acute stage in adolescents compared to adultsin terms of socio-demographic data, clinical presentation, cognitive function, and brain structural abnormalities with the same disorder.
Acute psychosis is a common psychiatric emergency that may present to health services other than mental health practitioners. Their response to antipsychotic drugs is very good and their outcome. The one-year prevalence of non-organic psychosis is 4.5 per 1000 community residents. Estimation of the prevalence of psychosis is 0.9 per 10, 000 at age 13years, 17.6 per 10, 000 at 18 years. The first episode of a psychotic disorder typically occurs in late adolescence or young adulthood, a critical time of development.
The first few years of psychosis appear to be a critical period especially in late adolescence and early adulthood during which early intervention needs to be initiated before the consequences of psychosis become more severe.
