Abstract

The purpose of this work was to assess the safety and the efficacy of multi-spot laser in comparison with the conventional laser in the treatment of non-proliferative diabetic retinopathy with CSME in relation to the visual outcome , central macular thickness by the optical coherence tomoghraghy (OCT) and the prescence of adverse events .
For this purpose 50 eyes divided into two groups each group was 25 eyes underwent either focal laser or modified macular grid using both conventional green laser (NIDEK GYC-1000 532nm) and multispot laser using the (VALON TT). BCVA, CMT, IOP, laser parameters and adverse events we measured before and after laser for follow up periods 1 week, one month and three months.
The BCVA improved in both groups as the mean preoperative BCVA in group A with conventional laser was 0.294 ± 0.1715 decimal and in group B with multispot laser BCVA was 0.3040 ± 0.2140. The mean BCVA 3 months post-laser in group A was 0.4820 ±0.244 decimal and in group B was 0.5080 ± 0.1977.there was no significant difference between two groups regarding improvement of BCVA (P >0.05).
The CMT improved in both groups as the mean preoperative CMT in group A was 375.92 ± 65.69 um and in group B was 361.0± 50.400 um. The mean CMT 3 months post-laser in group A was 314.44 ±85.938 um and in group B was 322.668 ± 57.500 um. There was no significant difference between two groups regarding improvement of CMT (P >0.05).
The intraocular pressure was not affected by laser as the mean preoperative IOP in group A was 16.32 ± 3.912 mmhg and in group B was 15.84± 3.923 mmhg. The mean IOP 3 months post-laser in group A
here was no significant effect of laser on IOP in two groups (P>0.05)
The laser parameters in the multispot laser was different. It used higher power (155 mW ± 90.1) than the conventional green laser (100 mW ± 19.4) and more number of spots 150 ± 88.3 compared to 80 ± 7.4 by conventional laser.
No complications related to laser treatment were noted in any patient. No effects were observed on blood vessels if the array inadvertently involved a retinal area traversed by blood vessels.
