Abstract

The child mortality is a composite index reflecting environmental, social, economic, health care services and delivery situation on one hand and maternal as well as family and community norms and practices on the other. A child is highly vulnerable to two categories of acquired illnesses; one is a heavy load of infectious diseases and the other, those diseases that are caused by inadequate nutrition. The relationship between child mortality and socio-economic factors might be relatively weak in developed countries, may be due to low child mortality. In contrast, in the developing countries, a significant portion of deaths occurs during childhood, which may be due to poor public health measures and lack of access to health care facilities.
The aim of the work was to study the most common causes and age groups of child mortalities in Alexandria University Children’s Hospital, El Shatby in the last five years.
This study was conducted on registries of mortalities that took place among inpatients of Alexandria University Children’s Hospital at El-Shatby from the 1st of January 2011 till the end of December 2015, including all deceased cases in Pediatric Department at Alexandria University Children’s Hospital except cases of neonatology unit.
The current work was a retrospective study, by reviewing the registers of all mortalities that took place among inpatients of Alexandria University Children’s Hospital.
Collected Data were: Age, Sex, Residence, Date of admission, Date of death, Duration of admission, Cause of death and PICU admission.
During the study period 101704 patients were admitted to Alexandria University Children’s Hospital, only 2237 cases of them died. The mortality percentage over the last five years ranged between 2.0 and 2.4%. There were 27 cases out of 2237 deceased cases whose data were incomplete or missing.
The mortalities according to year of death out of 2210 recorded deceased cases were minimum at 2011(18%) and were maximum at 2012 (22.3%).
The mortalities according to month of death were minimum in April (6.5%) while maximum in December (11.1%), while overall mortalities were (24.3%) in winter, (21.8%) in spring, (24.9%) in summer and (29%) in autumn.
The mortalities according to death age were 91.6% below age of 5 years while 8.4% were above age of 5 years.
According to the present data males (56.1%) were more than females (43.9%) and under-weight for their age were 43.1% of the studied cases.
According to residence, 42.8% were from Alexandria while 43.7% were from El-Behira and 13.5% were from other governorates.
According to cause of death, the hematological causes were the most common 501 cases (22.7%) while the CVS causes were the least common 209 cases (9.5%).
Only 15% of cases were admitted to PICU while 85% were not.
