Abstract

Patients with mammary hypertrophy present with variety of physical and psychological symptoms. The primary goal of breast reduction is symptomatic improvement of these patients. However there is another nearly equally important goal, which is creating a more aesthetic breast.
Many techniques have been described for reduction mammaplasty, each one having principles that are somewhat similar to or divergent from others. However there are critical common principles; sound surgical technique, knowledge of breast anatomy and preservation of perfusion. As breast reduction procedures have evolved, certain goals have been consistent. Among these are an aesthetic, natural breast shape and maintaining that breast shape long term as well as reducing scar problems. Of these goals, however, aesthetic breast shape remains of paramount importance; an operation that creates a breast with unacceptable shape, even with short scar, is not a success.
The aim of this study is to evaluate the clinical and aesthetic outcome of reduction mammaplasty procedure with superomedial pedicle versus inferior pedicle all with Wise-pattern skin resection.
The patients were divided into two groups:
 group I (10 patients): They were operated upon by the superomedial pedicle Wise-pattern technique.
 group II (10 patients): they were operated upon by the inferior pedicle Wise-pattern technique.
All patients were subjected to:
1- Preoperative detailed personal, family and past history taking.
2- Questionnaire about preoperative symptoms related to breast hypertrophy.
3- General examination and local breast examination.
4- Routine preoperative laboratory investigations and breast imaging.
5- Operative data were recorded and tabulated.
6- Complications were recorded and properly treated.
7- Photos were taken for every patient preoperatively and during the follow up period.
8- Discharge sheet containing date of discharge, hospital stay, condition of breast sensation (especially the NAC area) and complications.
9- Postoperative follow up after discharge from hospital after 1 week, 1month, 3 months and 6 months or more.
10- The following data were recorded during the follow up period which was done at 1, 3, 6 months up to 1 year.
