Abstract

Cystoid macular edema (CME) is one of the most frequent cause for preventable blindness in working adults in many countries. The beneficial effect of intravitreal ranibizumab injection in improving the visual acuity and retinal thickness in eyes with cystoid macular edema has been proved by many studies.
OCT added a high-resolution cross sectional scanning of the entire macular area. OCT can be fundamental in the diagnosis and follow up of CME, giving not only an objective measurement of the macular thickness with a resolution of 10 microns, but also an image of intraretinal structure and epiretinal tractions, This makes OCT the preferred indicator of therapeutic benefit of different treatment strategies.
The aim of this study was to evaluate the effect of intravitreal injection of ranibizumab (as a treatment for cystoid macular edema) on macular thickness using optical coherence tomography and the value of the OCT as a high resolution instrument for diagnosis and follow up of CME.
This study was carried out on 10 eyes of 10 patients. All of them have cystoid macular edema, all the cases received monthly intravitreal injection of ranibizumab along 3months and evaluated by OCT preinjection and one month after every injection.
Patients were excluded if they had macular hole or macular scar, any other ocular condition that might affect visual acuity during the course of the study (e.g corneal opacity or dense cataract.), neurosensory retinal detachment,
patient suffering from any reumatoid disease, patient received other intravitreal injection or patient with previous intraocular surgery as cataract operation.
Patients were subjected to detailed ophthalmic evaluation including anterior segment examination, intraocular pressure measurement, fundus examination with slit lamp biomicroscopy and macular thickness evaluation by OCT.
There was statistically significant decrease in CMT between preinjection and along the three monthly injections measured by OCT .
According to the current study CMT changes evaluated by OCT as follow :-
- After the 1st injection:- The mean CMT decreased significantly from 385.4±121.6 (197-528) pre injection to 313.4±110.1 (156-521) .
- After the 2nd injection:- continued the decrease of CMT to 283.2±128.2 (166-521).
- After the 3rd injection:- continued the decrease of CMT to 233.9±70.9 (158-417) .
