Abstract

Diabetic ketoacidosis (DKA) is an acute complication of type 1 diabetes (T1D). Previous studies have shown that DKA can be associated with the non-infectious form of the systemic inflammatory response and oxidative stress.
It has been suggested that serum BChE activity, mean platelet volume (MPV) and platelet distribution width (PDW) could serve as markers for low-grade systemic inflammation and oxidative stress occurring in DKA.
In this study, we aimed to measure serum BChE, MPV, and PDW in DKA and non-DKA patients, and their correlation to different clinical and laboratory diameters.
The present study was carried out on 60 diabetic children, 30 children admitted to our ER unit with DKA and 30 children already diagnosed as having diabetes mellitus Also, 30 apparently healthy children age and sex matched with the diabetic children were enrolled as control.
This study demonstrated that the activity of BChE and the values of MPV and PDW were significantly altered in patients with DKA compared with the values in non-DKA and control subjects. The ROC analysis showed that the three variables can equally predict the presence of DKA. But the elevated MPV can be a suitable risk marker for the diagnosis of DKA. Serum BChE activity, the MPV and PDW levels returned to normal after the commencement of insulin therapy.
In conclusion, our findings confirm the changes in BChE activity and platelet morphology in patients with DKA. And that the three variables can equally predict the presence of DKA. But the elevated MPV can be a suitable risk marker for the diagnosis of DKA.
