Abstract

Appendectomy is the most common surgical procedure performed in general surgery. For almost a century, open appendectomy, first described by charles McBurney in 1889, has remained the gold standard treatment for acute appendicitis.
The introduction of laparoscopic surgery has dramatically changed the field of surgery and laparoscopic surgery has been widely used as a minimally invasive surgery.
Laparoscopic appendectomy has emerged as a safe procedure with potential advantages including diagnostic and therapeutic values especially in females in their reproductive age.
This study was conducted to evaluate of the results of the laparoscopic versus open appendectomy in young females in regarding operative time, operative findings, post operative complications, hospital stay, post operative analgesia as well as return of the patients to their normal activity..
This study included 40 female patients who presented to Menoufia Faculty of medicine, Menoufia University, casuality department, from July 2015 to March 2016.The age of the patients ranged from 15-30 years and they were randomly allocated into two groups 20 patients each:
-Laparoscopic group : The patients will have laparoscopic appendectomy.
-Open group : The patients will have open appendectomy by gridiron incision at McBurney’s point.
The mean operating time in laparoscopic appendectomy was longer than that in the open approach (63.15 minutes in laparoscopic group versus 40.55 min in open group).
The overall post operative complications were diffirent in the 2 group. the incidence of wound infection was less in laparoscopic group(5% versus 15%). The mean hospital stay was shorter in laparoscopic patients than open appendectomy patients (1.9 days versus 3.3 days).
The post operative analgesia was less in laparoscopic than open group. The mean time to return to normal activities was shorter in laparoscopic appendectomy patients (5.1 days versus 8 days.).
Laparoscopy revealed 3 cases of gynecological causes of acute abdomen and they were managed laparoscopically without changing or extending incision, while in open group only one case was discovered and also managed with the same incision .
The use of laparoscope in suspected appendicitis is better than the open method especially in equivocal cases to reach an exact diagnosis. We must not hesitate to convert laparoscopic appendectomy to open appendectomy for the sake and safety of the patient.
