Abstract

Red cell distribution width (RDW)is a measure of circulating erythrocytes size heterogenecity,it is obtained routinely from automated complete blood count (CBC) some investigations showed that RDW could an important markers for cardiovascular disease (5-10).
The present study aimed to evaluate the possible relationship between (RDW) and extent of coronary artery disease in patient with ST segment elevation myocardium infarction (STEMI).
The study was conducted on 83 patients, with ST segment elevation myocardial infarction in national heart institute from November 2014 to September 2015. The patients were divided into two groups according to their RDW value as the following.
1-group I: RDW <14% and contain 36 patients.
2-group II: RDW>14% and contain 47 patients
Standard 12-lead ECG were performed for all patients,and blood sample were taken for analysis of RDW by (coulter) to evaluate CBC parameter and RDW according standerd method,normal reference range for RDW is (11.6-11.7%)(13) also lipid profile and routine lab were done.
Coronary angiography was done to asses coronary lesion and syntax sore and thrombus grading.
The current study found that RDW were higher in :
Diabetic, hypertensive ,dyslipidemic as RDW were higher with high triglyceride, cholesterol, and LDL Than non diabetic, normotensive, and normal lipid profile and the study was statistically significant.
Also the study found that patients with high RDW > 14 had higher rate of high syntax score ,high thrombus grading, rate of complication, and severity and numbers of diseased vessels than group I with statistically significant difference.
from all of the above RDW an inexpensive index that is routinely reported as a part of the complete blood count, RDW is an important marker for both diagnostic and prognostic purposes in various clinical cardiovascular settings.
