Abstract

Congenital adrenal hyperplasia is the most common cause of ambiguous genitalia in the newborn with an estimated incidence of 1 14000 live births. Most cases of virilized females with congenital adrenal hyperplasia present in the first month. The virilization degree is mostly Prader II, III, and IV. In our study, ten virilized females diagnosed as CAH due to 21 OH deficiency, between the ages of one month (SW 9 cases) to four years (SV 1 case), the diagnosis was done by pediatric endocrinologists in pediatric endocrinology unit. The virilization degree was III or IV. We were found that Ultrasonography in experienced hands is highly accurate in detection of the internal genital organs while, retrograde flush genitogram was less accurate and should be followed by MCUG for visualization of the site of confluence. Feminizing genitoplasty was done between the ages of six months to eight years, PUM with flap vaginoplasty was done to most of cases except one case a Passerini flap was needed only in one case. Anatomical and cosmetic outcome was compared with the most popular published criteria, Creighton criteria six months postoperative. The overall cosmetic results were 80% of cases were good, 10 % satisfactory and 10 % Poor which are a good results as compared with Creighton assessment We have two patients had developed vaginal stenosis and was treated with vaginal dilatation for 3 months with good urinary control in all patients.
