Abstract

Medication administration is the basic nursing function that involves skillful technique and consideration of the patient safety .drug may be a primary means of therapy for a patient with health problem, but any drug can cause harmful effect when administrated incorrectly. The nurse is responsible for understanding a drug`s action , and it`s site , drug`s name, preparation, factors affecting it`s action, monitoring the patient responses (Baldacci, J. & Harvy, B. 2010).
The aim of this study was to assess nurses’ knowledge, performance regarding to medication administration, and audit nurses ’documentation regarding to medication administration. This comparative study was carried out at El-Demerdash Hospital and Nasser Institute. It included 30 nurses from each of the two settings. The data were collected using a self-administered questionnaire sheet for nurses’ knowledge, an observation checklist for their performance. The tools were face and content-validated by a jury group of experts in nursing and medical administration. They were pilot-tested on 6 nurses to assess their clarity and applicability. The pilot also showed excellent reliability of the knowledge scale with a Cronbach alpha coefficient 0.933. The fieldwork lasted from started from November 2013 to the end of January 2014.
The main study findings were as follows.
- Nurses’ age ranged between The majority of the sample age has ranged from 40 to <50 years in El-Demerdash hospital while the majority of the sample in Nasser Institute hospital has ranged from 20 to < 30 years .
- As regarding to gender of sample almost all the nurses in the present study sample were females. As regarding to nurses marital status majority of the nurses were married in both As regards educational level, the majority of the current study sample holds diploma degree in nursing. While the minority of the sample holds bachelor degree Regarding the years of experience in the present study sample, the majority of the nurses were exceed ten years of experience in El-Demerdash hospital while less than half were exceed ten years of experience in Nasser institute hospital .
- More than half of the study sample have medication guidelines in their unit in El-Demerdash hospital. While the majority of the study sample have medication guidelines in their unit in Nasser institute hospital.
- Regarding the nurse client ratio during work distribution by case method, the study revealed that more than half of the nurses indicated that they deal with one nurse –to-from (2 to 5) clients in El-Demerdash . Also more than half from nurses in Nasser institute hospital indicated that they deal with one nurse –to-from (6-10) clients. Regarding the nurse client ratio during work distribution by function method, the study revealed that more than half of the nurses indicated that they deal with one nurse –to- from (3-10)clients in El-Demerdash .while at Nasser institute hospital the ratio ranged from one nurse –to- up to 19 client there are highly statistical significant differences between the nurse’s educational level and their knowledge in Nasser Institute hospital was p value less than 0.001.
There are highly statistical significant differences between the nurse’s years of experience and their knowledge in El-Demerdash was p value 0.008. The result indicates that is no statistically significant association in El-Demerdash hospital was p value less than 0.05 while there are statistical significant differences between nurses’ in Nasser Institute hospital was p value equal 0.046. Concerning the relation between nurses’ reading of procedure book for medication administration guidelines and their knowledge, The study results also indicated that there is significant difference between nurses in only El-Demerdash hospital. while no relation in nasser institute hospital.
Regarding to nurse’s performance and the results of the study showed that there are statistical significant differences between nurse’s performances and marital status, years of experience was p value less than 0.05. The results of the study showed in El-demerdash hospital no relation found due to poor performance for all sample subjects.
There are ahighly statistical significant differences between nurse’s performance and their knowledge .
As regarding to nurses documentation at nasser institute hospital the result indicates that is no statistically significant association with their nurses’ demographic characteristic and there documentation. There is no statistically significant association with their knowledge and nurses documentation. In nasser institute hospital the result indicates that is no statistically significant association with their knowledge and nurses documentation.
The result revealed that lowest level of satisfactory knowledge in Nasser institute was concerning the calculation of medication (23.3%), while in El-Demerdash Hospital it was the (3.3%).
 The highest satisfactory knowledge was those of at Nasser Institute and El-Demerdash hospitals (100%) concerning the high alert medication.
 Although more nurses in El-Demerdash Hospital had satisfactory knowledge (93.3%) compared with those in Nasser Institute (96.7%) the difference was not significant
 The mean scores of Nasser Institute nurses’ negative performance towards medication administration ranged between 2.5 for high risk medication and 70.5 for total medication administration during procedure.
 The percentages of inadequate performance in Nasser institute ranged between 50% for post procedure and 100% for administration of high risk medication. As for El-Demerdash Hospitals, it ranged between 96.7% for parentral medication and 100% for the other items .
 The performance of the nurses in E-Demerdash Hospital was significantly lower regarding all items (p>0.01) where as the performance of the nurses in Nasser Institute was significantly higher concerning the high risk medication.
 Overall, although more nurses in Nasser Institute had
un satisfactory total performance with percentage (93.3%) compared with those in El-Demerdash Hospital percentage (100%) the difference was high statistical significance (p>0.01) with mean 115.4 compared to El-Demerdash hospital 84.6.
 The mean scores of Nasser Institute nurses’ unsatisfactory performance regarding documentation of medication administration ranged between 20% toward documentation general criteria with mean 9.6 and 100% documentation in medication administration chart with mean 10.6 . for total 80% of nurses had unsatisfactory performance regarding documentation .
 The mean scores of El-Demerdash nurses’ unsatisfactory performance regarding documentation of medication administration ranged between 76.7% toward documentation of patient identification information with mean 8.3 and 100% documentation in medication administration chart and documentation in insulin chart with mean, 6.1, 1.3 respectively. for total 100% of nurses had unsatisfactory performance regarding documentation with mean 28.6
 The total unsatisfactory performance towards documentation of medication administration was higher among the nurses in El-Demerdash compared with those in Nasser Institute (45.3%), with high significant relation between them.
In conclusion, Although high percentage of the nurses in El-Demerdash Hospital and Nasser Institute have a satisfactory knowledge level regarding medication administration, but performance towards medication administration is significantly lower in El-Demerdash hospital particularly higher among nurses in Nasser institute than El-Demerdash Hospital , Also all nurses in El-Demerdash hospital had unsatisfactory performance regarding to documentation but majority of nurses in Nasser institute hospital had also unsatisfactory performance The important recommendation from the study results: were specific training for nurses in medication administration, calculation , medication documentation and with more emphasis on changing their performance. The hospital administration should provide nurses and other staff members with all the needed facilities and supplies necessary to fully implement medication administration procedure, Further research is proposed to assess the effect of nurses performance on quality of medication administration, patient outcomes, including patient satisfaction.
