Abstract

Objective: to evaluate the different surgical options for correction of patients with transposition of great arteries who presented lately and classical arterial switch operation is no longer valid for them, comparing the different options regarding their feasibility, efficacy, incidence of complications and reoperations for each.
Patients and methods: This study included 86 patients, who presented all patients with late TGA who took place in Geneva university hospital and Alexandria university hospital. from January 1989 until January 2013.
Results: Senning group, all patients in this group (77 patients) underwentSenning procedure. Pre-operative diagnosis included 15 patients with TGA with intact septum who had balloon atrial septostomy pre-operatively (22.1%), 14 patients TGA with ASD (18.9%), 10 patients TGA with ASD and PS (14.3%), 2 patients with TGA with VSD (2.7%), 9 patients with TGA with VSD with PS and BAS (12..2%), 7 patients with TGA and PDA and BAS (9.3%), 30 patients TGA with other pathologies. Cross clamp time ranged from 80 to 180 minutes with a mean of 113±22 minutes. 29 patients had a smooth post-operative course (37.7%), the remaining had events in their post-operative course. 90% discharged in sinus rhythm with overall survival was 86.1% at 24 years follow up. Mustard group, 9 patients underwent Mustard repair, and Pre-operative diagnosis was as following, 2 patients of TGA with intact ventricular septum with balloon atrial septostomy (22%), TGA with VSD, 2 patients (22%), TGA with ASD and PDA in 2 patients (22%), TGA with other pathologies in 3 patients (33.3%). Cross clamp time ranged from 55 to 120 minutes with the mean of 88±21 min. 4 patients had a smooth post-operative course (44%), and the remaining 5 had some complications in their post-operative course (56%). 89% discharged in sinus rhythm with overall survival of 100%.
