Abstract

This study aimed to compare OLO implant versus MMC as an adjuvant in combined trabeculotomy and trabeculectomy for treatment of primary congenital glaucoma.
This study was carried out during the period from April 2014 to October 2015. All surgeries were performed by four surgeons in Sohag University Hospital.
Thirteen eyes with PCG were included in this study. They were divided equally into two groups; OLO group (group A) and MMC group (group B).
All patients were subjected to preoperative evaluation starting with history taking from the parents; data collected were age, sex, main symptoms, family history and medical history. The patients were referred for full systemic evaluation by a pediatrician to rule out any associated systemic anomalies. In addition, they were examined under general anesthesia to measure corneal diameters and IOP level using Perkin’s applanation tonometer. Fundus examination was done if the corneal clarity permits.
Postoperative IOP level was divided into 4 subgroups in each main group:
Subgroup (I): Less than or equal to 14 mmHg
Subgroup (II): More than 14 to 16 mmHg
Subgroup (III): More than 16 to 20 mmHg
Subgroup (IV): More than 20 mmHg
In both groups; there was statistically highly significant reduction(P value <0.01) in IOP postoperatively and during follow up period.
Criteria of success were achieving target IOP level, improvement of corneal edema or haze, bleb function, stabilization in corneal diameters, improvement of symptoms and absence of postoperative complications.
Our results suggest that the collagen matrix implant(ologen)is a safe, and effective adjuvant in combined trabeculotomy and trabeculectomy for treatment of primary congenital glaucoma. Although there was insignificant difference between Ologen and MMC as regard efficacy, Ologen appears to be superior to MMC in certain aspects including: controlled IOP reduction, better bleb functioning, lower incidence complication especially postoperative hypotony.
Conclusion
