Abstract

Hypospadias is the most common congenital anomaly of male external genitalia. The incidence is approximately 1 in 250 live male births. The condition is characterized by ectopically located urethral meatus proximal to the normal location and on the ventral aspect of the penis. There’s no definitive cause for hypospadias. There are many factors that may account for it as testosterone deficiency, maternal use of oral contraceptive sduring early pregnancy, maternal exposure to antiepileptic drugs, intrauterine growth retardation and low birth weight, twins and advanced maternal age. Hypospadias can be classified according to location of the meatus into proximal and distal hypospadias. Distal presentation in which the meatus is located, glanular, coronal or subcoronal. And proximal urethral meatus represent perineal, scrotal, penos crotal or midshaft hypospadias. During the past two decades, we have witnessed considerable improvement in the functional and cosmetic results of hypospadias repair. This has been the result of a multitude of factors, including refinement of surgical technique, routine use of optical magnification, and better understanding of wound healing. There are many techniques described for hypospadias repair, until now there is no definitive technique used for all types of hypospadias. But the most popularized way that favored by many surgeons is the TIP urethroplasty, that is difficult and carry risk of more complications if performed in patients with narrow uretheral plate . Also patients with small glans that need glans augmentation are not fit for TIP procedure. In this work, we tried to evaluate the technique of double faced onlay island preputial skin flap in distal and mid penile hypospadias that advised in those patients with criteria don’t fit for TIP operation.
