Abstract

Stroke is one of the most common morbidities in elderly and it may be complicated with several impairments which can affect QoL. This spot light on the importance of developing health actions which add quality to life after the stroke.
This study was conducted to assess quality of life in elderly after cerebro vascular stroke using SF-36. It was a cross sectional study performed on 100 Patients 60 years and above with history of cerebro vascular stroke after three months from the last one.
Every patient was subjected to comprehensive geriatric assessment, detailed history taking in the form of personal history, demographic data, past medical history (including detailed history of stroke), complete clinical examination focusing on neurological examination, cognitive assessment using Mini-Cog, functional assessment using ADL and IADL. Mood assessment using GDS5 and assessment of quality of life by the Arabic version of short form 36 health survey.
Mean age of study population was 64.6±6.1 years, and most of them were males, married and illiterate.
The most prevalent comorbidity was hypertension followed by diabetes and least was thyroid disease and most of the cases had two comorbidities.
Left side weakness was the most affected side among studied cases and weakness was the most evident neurological deficit on clinical examination.
The most affected activity in ADL was bathing and the least was feeding while the most affected activity in IADL was laundry and housekeeping and the least affected activity was telephone use and medication.
The study showed that stroke affects QoL of elderly patients and the most affected domains were role limitation due to physical health and role limitation due to emotional problems.
QoL of stroke patients was negatively correlated to degree of dependency in ADL and IADL, depression, age, duration of stroke and number of comorbidities.
Most determinant factor for QoL domains and sub domains was age, while other factors were variable, such as mixed affection was the most determinant factor for emotional problems and total domains, also right affection was the most determinant factor for social function, physical function and general health, and DM was the most determinant factor for physical function, and duration of stroke was the most determinant factor for social function, pain and general health, while depression was the most determinant factor for emotional problems. And male gender was the most determinant factor for total domains, emotional problems, physical function and pain.
We concluded that HRQOL in elderly stroke patients is low, physical and emotional health is the mostly affected.
And we may recommend that HRQOL should be an important issue in assessment of elderly stroke patients and involved in their plan of management.
