Abstract

Subarachnoid hemorrhage is bleeding into the subarachnoid space without trauma.80% due to aneurysms other causes includes anticoagulants, A-V malformations, vacuities and tumuers.50%of patients die and only 50% of survivors return to normal life.it is presented by headache, vomiting, confusion, neck stiffness and convulsions. complications includes delayed cerebral ischemia(DCI), vasospasm and hydrocephalus. DCI is the most important cause of morbidity and mortality.it is diagnosed by CT brain, CT brain with angiography, MRI brain with FLAIR and lumber puncture. patient is managed in ICU hemodynamically and pharmacologically. Pharmacological medication includes calcium channel blocker nimodipine , intracranial nicardipine implant, endotheline 1 antagonist clazosentan, magnesium sulfate, nitric acid donor, statin, erythropoietin and fasudile. end result of SAH patient depends on severity of initial injury.
